【摘要】 目的探讨宫颈粒细胞肉瘤(GS)的病理特点、诊断与鉴别诊断及治疗与预后。方法观察1例原发性宫颈粒细胞肉瘤组织学形态及免疫组化,并复习相关文献,综合分析其临床病理特征及治疗与预后。结果本例首发症状为接触性出血、宫颈肥大、质硬。B超示宫颈处有一大小为2.5 cm×1.7 cm低回声占位肿块。光镜示瘤细胞呈弥漫性分布,由淋巴样细胞构成。瘤细胞小至中等大小,胞质少,核呈圆形或椭圆形,核分裂象易见。免疫组化:CD45、MPO、CD43、溶菌酶和CD68均(+),Ki-67阳性指数70%。骨髓及外周血无急性髓细胞白血病表现。结论宫颈粒细胞肉瘤十分罕见,初诊时易误诊为恶性淋巴瘤及小细胞性恶性肿瘤。免疫组化有助于诊断及鉴别诊断。更多还原

【Abstract】 Objective To study the clinical and pathologic features,diagnosis,differential diagnosis,treatment and prognosis of granulocytic sarcoma of the uterine cervix. Methods The clinical manifestations, histopathological featrures,immunohistochemistry,treatment and prognosis were analyzed in one case of granulocytic sarcoma of the uterine cervix with review of the literatures. Results One case of granulocytic sarcoma presented bleeding as a primary symptom,cervical hypertrophy and qualitative hard. B ultrasound prompted the cervix has a size of 2. 5 cm × 1. 7 cm low echo mass. Microscopically,the malignant cells grew in a diffuse pattern and composed of lymphoid cells. Tumor cells were small to medium in size. The nuclei were round and oval. The mitotic rates were visible. Histochemical stains for CD45,MPO,CD43,lysozyme and CD68 were positive. the proliferation index of Ki-67 was 70%. There was no acute myeloid leukemia in bone marrow and peripheral blood. Conclusions Granulocytic sarcoma of the uterine cervix is rare. In initial diagnosis,granulocytic sarcoma may be misdiagnosed as malignant lymphoma and small cell carcinoma; the diagnosis and differential diagnosis are based on immunohistochemical examination.更多还原
