【摘要】 患者女,63岁。左侧上颚部口腔黏膜褐色斑片3年,发生结节伴破溃6月余。皮肤科情况:左侧上颚部见一边界不清且不规则的淡褐色斑片,表面见一肤色结节。皮损组织病理示:黏膜上皮不规则增生,基底层黑素颗粒增多,真皮内交织排列的梭形细胞呈弥漫性增生,细胞核有异型改变,病变内散在色素颗粒。免疫组化检查示:Vimentin和S-100弥漫阳性,Ki-67约25%阳性。诊断:口腔黏膜梭形细胞黑素瘤。治疗:病变扩大手术切除,配合免疫疗法。更多还原

【Abstract】 A 63 years old female had a 3-year history of brown macule on the oral mucosa of hard palate,and with ulceration for 6 months. Physical examination showed a poorly and irregularly marginated brownish patch with a skin-colored nodule on the surface. Histopathology revealed irregular proliferation of the mucosal epithelium and increased melanin granules in basal layer. Interlaced bundles of spindle cells with little atypia appeared diffuse hyperplasia in the dermis. Pigment granules were scatteredly distributed in the lesion. Immunohistochemistry of tumor cells were diffusely positive for Vimentin and S-100,positive for HMB-45 and 25% positive for Ki-67. A final diagnose of spindle cell malignant melanoma was made. Extended resection with immunotherapy was given.更多还原
