武汉及其周边地区2011-2015年头癣类型及病原菌分析

【摘要】 目的了解近5年来武汉及其周边地区头癣临床特征及其病原菌分布情况。方法回顾性分析2011年1月-2015年12月本院皮肤科门诊诊治的头癣患者临床资料。结果 339例头癣患者,年龄50d~78岁,黑点癣209例(61.65%)、白癣81例(23.89%)、脓癣49例(14.45%),未发现黄癣。339例患者真菌培养出皮肤癣菌275例(81.12%),分别为紫色毛癣菌132株(48.00%)、犬小孢子菌69株(25.09%)、须癣毛癣菌31株(11.27%)、断发毛癣菌26株(9.45%)、红色毛癣菌10株(3.64%)、石膏样小孢子菌6株(2.18%)和铁锈色小孢子菌1株(0.36%)。结论武汉及其周边地区黑点癣发病率占首位,紫色毛癣菌为主要致病菌,犬小孢子菌、须癣毛癣菌、断发毛癣菌分列二、三、四位。

【Abstract】 Objective To investigate the clinical feature of tinea capitis and the distrubution of fungal pathogens in Wuhan and its rural areas in recent five years. Methods A retrospective analysis with tinea capitis from January 2011 to December 2015 was conducted at outpatient department of dermatology in our hospital. Results Among the 339 patients,from 50 days to 78 years old,there were black-dot ringworm 209( 61. 65%) cases;tinea alba 81( 23. 89%) cases,kerion 49( 14. 45%) cases and no tinea favosa. Fungal culture examination showed that a total of 275( 81. 12%) fungal strains were isolated from 339 cases,the distribution as follows:Trichophyton violaceum 132( 48. 00%) strains,Microsporum canis 69( 23. 89%) strains,Trichophyton mentagrophytes 31( 11. 27%) strains,Trichophyton tansurans 26( 9. 45%) strains,Trichophyton rubrum 10( 3. 64%) strains,Microsporum gypseum 6( 2. 18%) strains and Microsporum ferrugineum 1( 0. 36%)strains. Conclusions Black-dot ringworm was the main type of tinea capitis in Wuhan and its surrounding areas,and Trichophyton rubrum was the most common pathogenic fungi,Microsporum canis,Trichophyton mentagrophytes and Trichophyton tansurans ranked the second,third and fourth respectively.
