深海鱼油对2型糖尿病老年患者糖脂代谢影响:一项双盲随机对照研究
EFFECT OF DEEP SEA FISH OIL ON GLUCOSE AND LIPID METABOLISM IN ELDERLY PATIENTS WITH TYPE 2 DIABETES MELLITUS:A DOUBLE-BLIND RANDOMIZED PLACEBO-CONTROLLED TRIAL
【摘要】 目的探讨富含n-3多不饱和脂肪酸的鱼油对老年2型糖尿病患者糖脂代谢的影响,为n-3多不饱和脂肪酸在2型糖尿病治疗中的应用提供科学依据。方法采用双盲随机对照的研究方法,将筛选的156名研究对象随机分成深海鱼油干预组和对照组,进行为期6月的双盲干预研究,探讨n-3多不饱和脂肪酸对糖脂代谢的影响。结果 3月后干预组的内脏脂肪等级较对照组显著减小(P=0.049),干预组血清甘油三酯水平显著低于对照组(P=0.019),高密度脂蛋白水平显著高于对照组(P=0.039),TG/HDL值显著低于对照组(P=0.007);6月后,干预组血清空腹胰岛素降低值显著高于对照组(P=0.043),干预组定量胰岛素敏感性指标数水平有一定程度的升高,但无统计学意义;干预组血清甘油三酯水平显著低于对照组(P=0.019),高密度脂蛋白水平显著高于对照组(P=0.004),TG/HDL值显著低于对照组(P=0.021)。结论 n-3多不饱和脂肪酸能调节2型糖尿病老年患者的脂代谢,对胰岛素敏感性和内脏肥胖有一定的改善作用。
【Abstract】 Objective The study was aimed to investigate the effect of n-3 PUFA on glucose and lipid metabolisms in elderly patients with T2DM. Methods The intervention study was a double-blind randomized placebo-controlled trial. A total of 156 subjects(aged≥60) were included in the study, and divided into fish oil group and corn oil group randomly. They received interventions for 6 months to investigate the effect of n-3 PUFA on glucose and lipid metabolisms. Results After 3-month intervention, n-3 PUFA decreased visceral fat level significantly compared with control group(P=0.049). The variation value of fasting insulin of intervention group increased significantly compared with control group(P=0.043), and increased quantitative insulin sensitivity check index(QUICKI) moderately, but no statistical significance. Serum TG level decreased significantly by intervention(P=0.019 vs. placebo), serum HDL increased significantly(P=0.039 vs. placebo). and TG/HDL decreased significantly(P=0.007 vs. placebo).After 6-month intervention, serum TG level decreased significantly(P=0.019 vs. placebo). Serum HDL increased significantly(P=0.004 vs. placebo), and TG/HDL decreased significantly(P=0.021 vs. placebo). Conclusion: n-3 PUFA improved lipid metabolism in elderly patients with T2DM, and may be effective in improving insulin sensitivity and visceral obesity.
