基于显微超声技术根管治疗对钙化根管患者疗效的影响
Effect of Root Canal Therapy Based on Micro Ultrasound Technique on the Efficacy of Calcified Root Canal
【摘要】 目的探讨显微超声技术对钙化根管患者疗效的影响。方法连续选择明确诊断钙化根管需行根管治疗的40例患者的患牙43颗(46个钙化根管),在手术显微镜下采用超声工作尖和(或)超声根管锉去除根管内钙化组织,同时结合乙二胺四乙酸(EDTA)、小号手用锉疏通根管,机用镍钛器械Pro Taper完成根管预备,侧向加压或热牙胶垂直加压技术充填根管。按照病变牙位及钙化部位分别记录钙化根管疏通成功率及根管内并发症的发生情况。结果磨牙组钙化根管治疗总有效率显著低于前牙组(P<0.05)。根管上1/3钙化组患者治疗总有效率显著高于根管中1/3组及根管下1/3组患者(P<0.05)。10个钙化根管未能成功疏通。结论显微超声技术是处理钙化根管的有效方法,尤其针对前牙及根管口及上段再通成功率较高,但对于根管下段及弯曲部位病变疗效欠佳。
【Abstract】 Objective To detect the effect of root canal therapy based on micro ultrasound technique on the efficacy of calcified root canal. Methods Continuous selection diagnosis of calcified root canal which need root canal therapy in 40 patients with 43 teeth(46 root canal calcification), using ultrasonic tips and under surgical microscope(or) ultrasonic root canal file removal of calcified tissue in the root canal, combined with ethylen ediaminetetraacetic acid(EDTA), with a file to clear the root canal the little bugler, machine complete with Pro Taper Ni Ti instruments in root canal preparation, lateral or vertical compaction technique in root canal filling. The success rate of calcified root canals and the complications in the root canals were recorded according to the location of the diseased teeth and the calcification sites. Results The total effective rate of molar root canal calcification treatment was significantly lower than that of the anterior teeth group. The total effective rate of 1/3 calcification group in root canal was significantly higher than that in 1/3 group and 1/3 group under root canal. Conclusion Micro ultrasonic technique is an effective method for treatment of calcified root canals, especially for the anterior teeth and the root canal and the upper segment. The success rate is higher, but the lower part of the root canal and the curved part are less effective.
