凹陷性颅骨缺损及其颅内压变化的研究进展
Research Progress of Intracranial Pressure Change in Depressed Skull Defect
【摘要】 凹陷性颅骨缺损由多种原因引起,但以标准去大骨瓣减压术后多见。它打破了颅腔内原有的生理平衡,造成颅内压的改变。这种颅内压力变化受到大气压力、患者体位、脑组织自身的重力及颅骨缺损面积的影响。颅内压力的变化会引起脑脊液及脑血液循环动力学紊乱,造成脑血流灌注不足,导致大脑代谢紊乱及潜在的脑组织功能损害。尽早恢复颅腔的完整性,维持颅内压力的平衡至关重要。因此,缺损颅骨的重建时机选择一直是神经外科医师探索的课题。
【Abstract】 Depressed skull defect is caused by a variety of causes,especially after the standard large bone flap decompression. Depressed skull defect breaks the physiological balance of the original cavity,resulting in the change of intracranial pressure. The changes of intracranial pressure are affected by atmospheric pressure,body position,the gravity of the brain tissue and the area of the skull defect. The changes of intracranial pressure can cause dynamic disorders of cerebrospinal fluid circulation and cerebral blood circulation,resulting in cerebral blood flow perfusion deficiency,leading to brain metabolic disorder and potential brain tissue damage. Recovery as soon as possible to complete the cranial cavity is vital to maintain intracranial pressure balance. Therefore,the timing of skull defect reconstruction has always been a topic for the neurosurgeons to explore.
