绵阳市中心医院2010—2014年临床分离病原菌的分布特征及耐药性分析

【摘要】 目的了解绵阳市中心医院2010—2014年病原菌的分布特征及耐药性变迁,为临床治疗提供参考依据。方法采用微生物全自动分析仪与手工相结合的方法对2010年1月1日至2014年12月31日绵阳市中心医院临床标本中分离出的病原菌进行鉴定和药敏试验,采用Whonet 5.6软件进行统计分析。结果 5年来共分离出病原菌27 825株,其中革兰阴性菌18 947株,以大肠埃希菌居最多共4 997株,占26.4%,其次为肺炎克雷伯菌3 638株,占19.2%,铜绿假单胞菌2 301株,占12.1%,鲍曼不动杆菌2 263株,占11.9%。革兰阳性细菌5 910株,金黄色葡萄球菌为主2 514株,占42.5%。标本种类主要是痰、血液、尿液和切口分泌物等;标本主要来源于呼吸内科、儿科、普外科和ICU等科室。金黄色葡萄球菌中耐甲氧西林金黄色葡萄球菌占17.2%~21.9%;肠杆菌科细菌对亚胺培南、厄他培南和阿米卡星的敏感性好;鲍曼不动杆菌对亚胺培南敏感性差;复方新诺明对不同的细菌敏感性差异较大,需根据药物敏感结果来判断用药。结论定期进行细菌耐药性监测有助于了解本医院细菌耐药情况,为临床经验用药提供理论依据。

【Abstract】 Objective To investigate the tendency of antibiotic resistance and distribution of pathogens isolated from Mianyang Central Hospital during 2010-2014 and to provide reference for the clinical treatment.Methods All pathogenic bacteria were collected in Mianyang Central Hospital from Jan.2010 to Dec.2014,and were detected by automatic microbiology analyzer and manual method,and the data were analyzed by Whonet 5.6.Results A total of 27 825 bacter ia were isolated in 5 years,18 947 of them were gram-negative bacteria,the isolates were mainly Escherichia coli(26.4%),followed by Klebsiella pneumoniae(19.2%),Pseudomonas aeruginosa(12.1%) and Acinetobacter baumannii(11.9%);5 910 of them were gram-positive bacteria,the majority were Staphylococcus aureus(42.5%).The main types of specimen were sputum,blood,urine and secretion.The specimens were mainly from respiratory department of internal medicine,department of general surgery,department of pediatrics and ICU.The incidences of meticillin-resistant Staphylococcus aureus were 17.2%-21.9%; Enterobacteriaceae were sensitive to imipenem,ertapenem and amikacin; Acinetobacter baumannii were not sensitive to imipenem.The sensitivity to compound sulfamethoxazole varied a lot per different bacteria,it’s necessary to determine the medication according to drug sensitivity test.Conclusion It is helpful to understand the bacterial drug resistance in the hospital by regular surveillance of bacterial resistance,to provide reference for the clinical use of antimicrobial agents.
