声速匹配联合多普勒参数对慢性乙型肝炎肝纤维化评价的初步研究

【摘要】 目的:探讨声速匹配技术(Sound speed correction,SSC)联合多普勒超声(doppler ultrasonography,DU)在评估慢性乙型肝炎肝纤维化诊断及分期的准确性。方法:选取经临床诊断诊的慢性乙型肝炎患者137例作为研究组,同期健康志愿者21例作为对照组。应用SSC技术测定研究对象的肝组织区域速度指数(zone speed index,ZSI)及肝固有动脉的血流动力性参数(收缩期峰值流速PSV、舒张末期流速EDV及阻力指数RI),根据肝组织病理类型将研究组分为肝纤维化S0、S1、S2、S3、S4组,比较纤维化组(S1~S4)与无纤维化组(S0+对照组)的ZSI及多普勒参数在慢性乙型肝炎肝纤维化诊断中的价值。结果:以ZSI=21.88m/s为截断值,诊断肝纤维化的灵敏性、特异性分别为81%、75%,以PSV=72.16cm/s为截断值,诊断肝纤维化的灵敏性、特异性分别为87%、61%;以RI=0.64为截断值,诊断肝纤维化的灵敏性、特异性分别为87%、56%;ZSI联合多普勒参诊断肝纤维化的灵敏性、特异性分别为96%、89%。结论:SSC联合多DU可提高肝纤维化诊断及分期的准确性。

【Abstract】 Objective To evaluate the accuracy of Sound speed correction(SSC) combined with Doppler ultrasonography in the diagnosis and staging of liver fibrosis in chronic hepatitis B. Methods One hundred and thirty severn patients with chronic hepatitis B diagnosed by clinic were selected as the study group, and twenty one healthy volunteers as control group at the same time. All cases of the study and control groups were evaluated the zone speed index(ZSI) and proper hepatic artery paramentes(PSV, EDV and RI). According to the liver fibrosis histopathology, The patients of study group were divided into fibrosis group and non-fibrosis group and all the paramenters were compared in the each group. Results The cut-off value of ZSI as 21.88 m/s, the diagnostic sensitivity and specificity of liver fibrosis were 81% and 75%, respectively. The cut-off value of PSV of proper hepatic arter as 72.16 cm/s, the diagnostic sensitivity and specificity of liver fibrosis were 87% and 56%, respectively. The cut-off value of RI of proper hepatic arter as 0.64, the diagnostic sensitivity and specificity of liver fibrosis were 87% and 56%, respectively. Conclusion SSC combined with Doppler ultrasonography improves the non-invasive ssessment of fibrosis in patients with in chronic hepatitis B.
