基于数学模型的高危HPV E6/E7 mRNA定量分析在宫颈病变筛查中的作用

【摘要】 目的:研究高危人乳头瘤病毒(HPV)E6/E7 mRNA定量分析在宫颈病变筛查中的作用。方法:采用原位杂交技术对440例不同宫颈病变[炎症、低级别上皮内瘤变(CIN)、高级别CIN、浸润性鳞癌]患者行高危HPV E6/E7 mRNA拷贝数进行定量检测,建立数学模型对检测结果进行分析。结果:4组之间HPV E6/E7 mRNA阳性率不全相等,具有统计学差异(P<0.005 0);炎症组与其他3组宫颈病变之间高危HPV E6/E7 mRNA阳性率均有统计学差异(P<0.008 3)。拷贝数大小与病变程度呈正相关,可知当mRNA小于500时,炎症的可能性比较高;500~10 000时,很可能是CIN(包括低级别和高级别);大于10 000时,很可能是高级别CIN和癌症。结论:高危HPV E6/E7 mRNA拷贝数大小与宫颈病变程度正相关,其定量检测可作为宫颈病变联合筛查的一种有效补充工具。

【Abstract】 Objective To study the value of quantitative analysis of high-risk human papilloma virus(HPV) E6/E7 mRNA in cervical lesion screening. Methods The copies of high- risk HPV E6/E7 mRNA were quantitatively detected using in- situ hybridization technique in 440 patients with different cervical lesions, including inflammatory lesions, low- grade cervical intraepithelial neoplasia(CIN), high-grade CIN, squamous-cell carcinoma. A mathematic model was used to analyze the results of the tests. Results The positivity rates of high-risk HPV E6/E7 mRNA differed significantly among the 4 groups(P<0.005).The positivity rates of high-risk HPV E6/E7 mRNA showed significant differences between the inflammatory lesions and the other 3 lesions(P<0.008). The copies of high- risk HPV E6/E7 mRNA were positively correlated with the severity of the lesions, and a mRNA copy number less than 500 suggested a high probability of inflammatory lesions, whereas a copy number of 500- 10 000 indicated a likely diagnosis of CIN(including low- and high- grade ones); a mRNA copy number exceeding10 000 may support the diagnosis of high-grade CIN or cancer. Conclusion The copy number of high-risk HPV E6/E7 mRNA is positively correlated with the pathological grade of the cervical lesion. Quantitative detection of high- risk HPV E6/E7 mRNA can serve as a complementary examination for cervical lesion screening.
