二氧化碳气腹对新生儿脑氧供需及代谢的影响观察

【摘要】 目的分析二氧化碳气腹对新生儿脑氧供需及代谢的影响。方法选取30例于我院行腹腔镜手术的新生患儿,测定气腹对血脑氧供需及代谢的影响。结果 CO2气腹中血液颈静脉血氧饱和度(SjvO2)升高,脑动-静脉血氧含量差(Da-jvO22)与脑氧摄取率(COER)则出现降低(P<0.05);脑动-静脉血糖含量差(Da-jvGlu)与葡萄糖摄取率(GluER)显著降低,颈静脉乳酸(Jlac)水平显著增高(P<0.05)。气腹解除后以上指标逐渐恢复正常。结论腹腔镜手术中CO2气腹可影响新生儿脑氧供需平衡以及脑组织能量代谢水平,但这些影响是暂时并可逆的。

【Abstract】 Objective To analysis the effects of CO2 pneumoperitoneum on cerebral oxygen supply and demand and metabolism in neonates.Methods Select 30 cases of children’s hospital in Jiangxi Province underwent laparoscopic surgery in newborns to investigate the effect of pneumoperitoneum on blood supply and metabolism of blood. Results The blood oxygen saturation(SjvO2)of blood j μgular vein increased,and brain venous blood oxygen content difference(Da-jvO2)and cerebral oxygen uptake rate(COER)decreased during CO2 pneumoperitoneum(P<0.05).In addition,brain-venous blood glucose difference(Da-jvGlu)and glucose uptake rate(GluER)were significantly decreased,and Jlac levels were significantly increased(P<0.05).After the release of pneumoperitoneum gradually returned to normal indicators.Conclusion Laparoscopic operation of CO2 pneumoperitoneum can affect neonatal cerebral oxygen supply and demand balance and brain tissue energy metabolism,but these effects are temporarily reversible.
