螺旋CT三维成像在颌面部复杂骨折中的应用

【摘要】 目的研究螺旋CT三维成像在颌面部复杂骨折中的价值。方法纳入我院收诊的70例疑似颌面部复杂骨折患者,均经常规CT扫描和螺旋CT三维成像检查,以手术病理结果为金标准,评价两种方法在颌面部复杂骨折诊断中的价值。结果经手术病理证实,颧骨合并上颌骨骨折15例,髁骨骨折9例,粉碎性上下颌骨骨折10例,筛骨骨折8例,全面部骨折14例,颧骨颧弓骨折12例,无骨折2例。常规CT扫描与螺旋CT三维成像检出率均为100.00%,组间无显著性差异(P>0.05)。螺旋CT三维成像准确率为92.86%,略高于常规CT扫描的82.86%(P>0.05)。结论螺旋CT三维成像应用于颌面部复杂骨折能清晰显示骨折立体结构,对诊断骨折类型具有较高的准确性,但不能完全替代常规CT二维扫描。

【Abstract】 Objective To study the application value of spiral CT three-dimensional imaging in complex maxillofacial fractures. Methods Seventy cases of patients with suspected complex maxillofacial fractures diagnosed in our hospital were enrolled, and examined by routine CT scan and spiral CT three-dimensional imaging. The results of surgical pathology were selected as the gold standard to evaluate the value of two kinds of examination methods in the diagnosis of complex maxillofacial fractures. Results The surgical pathology confirmed that there were 15 cases of zygomatic maxillary fracture, 9 cases of condylar bone fracture, 10 cases of comminuted upper and lower jaw fractures, 8 cases of ethmoid fractures, 14 cases of panfacial fractures, 12 cases of zygomatic fracture and 2 cases of no fracture. The fracture detection rate of the conventional CT scan and spiral CT three-dimensional imaging were both 100.00%, there was no significant difference between the two methods(P>0.05). The accuracy of spiral CT 3 D imaging was 92.86%, which was slighterly higher than82.86% of the conventional CT scan(P >0.05). Conclusion The spiral CT 3 D imaging can clearly show the three-dimensional structure of fractures in maxillofacial complex fractures, which has high accuracy in the diagnosis of fracture types, but can’t completely replace the conventional CT two-dimensional scanning.
