【摘要】 目的研究前列腺癌经去势治疗后出现去势抵抗与全身炎症指标相关性。方法回顾性分析199例接受内分泌治疗的前列腺癌患者的临床资料。根据其出现去势抵抗时间将其分为<1年、1~3年、3~5年复发和5年内未复发四组。分析各组患者穿刺诊断前总前列腺特异性抗原(tPSA)、C反应蛋白(CRP)、白蛋白(Alb)、改良格拉斯哥预后评分(mGPS)、临床分期、Glasgow评分以及是否存在骨转移与患者出现去势抵抗时间的相关性。结果各组患者的临床分期、Gleason评分、tPSA、CRP、Alb及mGPS均有统计学差异(P<0.05)。患者术前mGPS、CRP及Gleason评分越高,在1年和3年内发生去势抵抗的风险越高。多因素Logistic回归分析发现,CRP>10mg/L、Gleason评分≥8分和mGPS≥1分为前列腺癌出现去势抵抗进展时间的独立危险因素(P<0.05)。结论全身炎症指标可能可作为预测前列腺癌出现去势抵抗进展时间的独立危险因素。更多还原

【Abstract】 Objective To study the association between systemic inflammation biomarkers and castration-resistance of prostate cancer.Methods Clinical data of 199 patients with prostate cancer underwent prostate resection were retrospectively analyzed.According to the occurrence time of castration-resistance,the patients were assigned into four groups of A(<1year),B(1-3years),C(3-5years)and D(no castration-resistance within 5years).Clinical features including total prostate specific antigen(tPSA),C-reactive protein(CRP),albumin(Alb),modified Glasgow prognostic score(mGPS),clinical stage and Gleason score were compared among four groups.Results There were significant differences in clinical stage,Gleason score,tPSA,CRP,Alb and mGPS among four groups(P<0.05).The higher levels of CRP,mGPS and Gleason score,the greater risk for the occurrence of castrationresistance within 1year and 3years.Multivariate analysis showed that CRP>10mg/L,Gleason score≥8points and mGPS≥1point were the independent risk factors for the occurrence time of castrationresistance(P<0.05).Conclusion Systemic inflammation biomarkers can be taken as the risk factors for predicting the occurrence of castration-resistance in the patients with prostate cancer.更多还原
