【摘要】 目的观察联合应用多西他赛、奥沙利铂和替吉奥(DOS)新辅助化疗治疗进展期胃上部癌和胃下部癌的疗效。方法进展期胃癌患者72例分为胃上部癌(UGC组,29例)和胃下部癌(LGC组,43例)两组,均采用DOS方案新辅助化疗3个周期,4周后行标准D2手术,术后常规化疗。比较两组疗效和不良反应等相关指标。结果两组化疗不良反应发生率相仿(P>0.05),且多为Ⅰ、Ⅱ级反应,经对症治疗后均有所缓解;其不良反应主要为白细胞减少、血小板减少、食欲下降和恶心呕吐等。UGC组的临床有效率和疾病控制率均高于LGC组(82.8%vs.60.5%和100%vs.86.0%)(P<0.05)。两组患者1年生存率比较无统计学差异(82.6%vs.83.5%)(P>0.05)。结论DOS方案新辅助化疗治疗进展期胃上部癌的疗效较进展期胃下部癌更好。更多还原

【Abstract】 Objective To observe the clinical efficacy of neoadjuvant chemotherapy with combined use of docetaxel,oxaliplatin and tegafur gimeracil oteracil potassium capsule(S-1)(DOS regime)in the treatment of advanced gastric carcinoma located at upper and lower part of stomach.Methods A total of 72 patients with advanced gastric carcinoma was divided into two groups of UGC(the carcinoma located at upper part of stomach,29cases)and LGC(the carcinoma located at lower part of stomach,43cases).The patients of groups were treated with neoadjuvant chemotherapy of DOS regime for three cycles.The standard D2 surgery was performed 4 weeks after neoadjuvant chemotherapy.The routine chemotherapy was carried out after surgery.Clinical efficacy and adverse effects related to neoadjuvant chemotherapy were compared between two groups.Results The incidence of adverse effects in both groups was similar(P>0.05).The main adverse effects were leukopenia,thrombocytopenia,loss of appetite,nausea and vomiting,which were not severe and attenuated by symptomatic treatments.The overall response rate and disease control rate were higher in group UGC than those in group LGC(82.8% vs.60.5% and 100% vs.86.0%)(P<0.05).There was no significant difference in 1-year survival rate between two groups(82.6% vs.83.5%)(P>0.05).Conclusion The clinical efficacy of neoadjuvant chemotherapy with DOS regime is better in treating the advanced carcinoma located at upper part of stomach than that located at lower part of stomach.更多还原
