【摘要】 目的:探索区域医疗卫生服务共同体分级诊疗创新模式下,建立慢性病管理和双向转诊及数据共享机制,促进卫生资源的合理利用,降低医疗费用,提高患者满意度。方法:以西安交通大学第一附属医院与陕西省卫生和计划生育委员会在西安市雁塔区进行分级诊疗为试点单位,建立基于慢病管理和双向转诊的区域分级诊疗信息化建设模式。结果:制定了全省分级诊疗转诊单信息化标准,开创了全省第一家大规模分级诊疗信息化的创新实践,慢病管理使用初具规模,双向转诊持续推进。结论:通过雁塔区慢病管理和双向转诊分级诊疗的实施及其信息化建设,为陕西省分级诊疗的建设和推广应用提供实践经验。更多还原

【Abstract】 Objective:To explore the regional health service community under hierarchical diagnosis and treatment mode,the establishment of chronic disease management and two-way referral system,realize the data sharing,promote the rational utilization of health resources,reduce the medical costs and improve satisfaction of patients.Methods:With the First Affiliated Hospital of Xi’an Jiaotong University and Shaanxi Health and Family Planning Commission for hierarchical diagnosis and treatment as the pilot unit in Yanta District of Xi’an City,to establish the chronic disease management and referral area grading treatment mode based on information construction.Results:The province’s grading clinic referral information standards was developed,the first large-scale classification of medical information was created and innovative practiced,chronic disease management was gradually shaped up,which was continually promoted for the two-way referral.Conclusion:Through the management of chronic diseases and two-way referral treatment and the information construction in Yanta District,it provided the practical experience for Shaanxi Province in the construction and application of hierarchical diagnosis and treatment.更多还原
