实时超声弹性成像硬度分级法联合弹性应变率比值鉴别诊断腮腺肿块的对照研究
Comparative Study on Real-Time Ultrasonic Elastography Hardness Classification Combined with Elastic Strain Rate Ratio in the Differential Diagnosis of Parotid Mass
【摘要】 目的探讨实时超声弹性成像硬度分级法联合弹性应变率比值鉴别诊断腮腺肿块的价值。方法选取我院收治的92例腮腺肿块患者,先行常规检查,再给予实时超声弹性成像检查,采用5分法评估,同时采用超声诊断仪测量各肿块的弹性应变率比值,依据约登指数最高临界点确定弹性应变率比值诊断界点,并与病理结果进行对照比较。结果实时超声弹性成像硬度分级法诊断腮腺良恶性肿块的特异性、敏感性、准确性分别为88.0%、83.4%、87.0%,明显低于实时超声弹性成像硬度分级法联合弹性应变率比值的96.8%、89.2%、93.5%,差异有统计学意义(P<0.05)。结论实时超声弹性成像硬度分级法联合弹性应变率比值有助于腮腺肿块的鉴别诊断,具有临床价值。
【Abstract】 Objective To explore the value of real-time ultrasonic elastography hardness classification combined with elastic strain rate ratio in the differential diagnosis of parotid mass. Methods 92 cases of patients with parotid mass admitted to our hospital were selected,and given routine examination firstly, then the real-time ultrasonic elastography. 5-point method was used to evaluate. The elastic strain rate ratio of masses was measured by ultrasonic instrument. The diagnostic boundary point of elastic strain rate ratio was determined according to the highest Youden index critical point, and was compared with pathological results. Results The specificity, sensibility and accuracy of real-time ultrasonic elastography hardness classification in the diagnosis of benign and malignant parotid mass were respectively 88.0%,83.4% and 87.0%, significantly lower than 96.8%, 89.2% and 93.5% of real-time ultrasonic elastography hardness classification combined with elastic strain rate ratio, with statistical differences(P <0.05). Conclusions Real-time ultrasonic elastography hardness classification combined with elastic strain rate ratio helps for differential diagnosis of parotid masses and has clinical value.
