磁共振弥散加权成像动态评价唾液腺功能的初步研究

【摘要】 目的动态观察酸刺激前后腮腺和颌下腺MR-DWI ADC值的变化规律,探讨MR-DWI评价唾液腺功能的可行性。方法选择2011年78月在福建省肿瘤医院首诊的健康受试者10例作为研究对象,分别在静息状态下和酸刺激后不同时间点进行唾液腺MR-DWI扫描,观察酸刺激前后不同时间点腮腺和颌下腺MR-DWI ADC值的变化规律。结果10例受试者静息状态下腮腺和颌下腺的ADC值分别为(0.95±0.06)×10-3mm2/s和(1.20±0.10)×10-3mm2/s,二者对比存在显著性差异(t=6.203,P<0.001);腮腺和颌下腺在酸刺激后的第3min ADC值均升高,第6min ADC值明显下降,此后呈逐渐上升,酸刺激后1521min达高峰平台水平,然后逐步下降至静息状态水平。结论 MR-DWI通过ADC值的量化,可动态观察到不同生理状态下唾液腺功能的变化。

【Abstract】 Objective To observe the dynamic changes in MR-DWI ADC values of parotid and submandibular gland before and after acid stimulation,and to further explore the feasibility of MR-DWI in salivary gland function evaluation.Methods A total of 10 healthy volunteers in Fujian provincial tumor hospital between July and August 2011 were recruited,MR-DWI scanning was done at different time periods before and after acid stimulation for each patient,then the ADC values of salivary glands were recorded.The changes of MR-DWI ADC value of parotid and submandibular gland before and after acid stimulation were analyzed.Results In 10 patients,the ADC value of parotid gland and submandibular gland in resting state was( 0.95±0.06) ×10-3mm2/s and( 1.20±0.10) ×10-3mm2/s,respectively.The ADC value of salivary gland was increased in first 3 minutes after acid stimulation,then decreased at 6th minute,and then increased gradually to the peak level at 15 th 21 st minutes,and then gradually declined to resting levels.Conclusion MR-DWI can noninvasively evaluate the dynamic physiologic changes of salivary glands before and after acid stimulation.
