家族性渗出性玻璃体视网膜病变合并视网膜脱离一例

【摘要】 家族性渗出性玻璃体视网膜病变(FEVR)为引起青年性视网膜脱离的主要病因之一,眼底荧光素造影(FFA)对FEVR合并视网膜脱离的早期诊断及治疗具有重要意义,联合抗血管内皮生长因子(VEGF)治疗可增加视网膜脱离手术成功率。该文报道1例以左眼视物不清为首要症状的青年患者,根据裂隙灯检查,联合FFA、光学相干断层扫描、彩超等辅助检查确诊为FEVR合并视网膜脱离。初期检查时未见明确裂孔,根据患者左眼玻璃体色素颗粒及视网膜下可见增殖条索等特征,判断其合并孔源性视网膜脱离。于玻璃体腔注射抗VEGF药物,再次详细检查眼底,发现可疑裂孔,行巩膜外冷凝环扎外垫压术治疗,术后视网膜复位良好。该例提示在FEVR患者合并视网膜脱离时应详细检查裂孔,抗VEGF药物可辅助临床诊断。

【Abstract】 Familial exudative vitreoretinopathy(FEVR) is one of the main causes of juvenile retinal detachment. Fundus fluorescein angiography( FFA) is of great significance to the early diagnosis and treatment of FEVR complicated with retinal detachment. FFA combined with anti-vascular endothelial growth factor(VEGF) therapy can increase the success rate of retinal detachment surgery. In this article,we reported a juvenile patient presenting with blurred vision of the left eye as the primary symptom. The patient was eventually diagnosed with FEVR complicated with retinal detachment after slip-lamp examination,FFA,optical coherence tomography( OCT) and color Doppler ultrasound. During the initial examination,no holes were detected. According to the characteristics of the pigmented granules of the vitreous body of the left eye and the sub-retinal proliferative band,the patient was diagnosed with rhegmatogenous retinal detachment. After intravitreal injection of anti-VEGF agents and explicit fundus examination,scleral cryotherapy combined with scleral buckling surgery was performed. The retina was properly re-attached after surgery. This case prompts that the holes should be explicitly examined in FEVR patients complicated with retinal detachment,and anti-VEGF agents can be utilized to assist clinical diagnosis.
