自体心包片瓣环成形技术预防瓣膜替换手术瓣周漏的效果分析

【摘要】 目的探讨瓣膜替换手术中瓣周漏发生的原因,解剖因素和使用自体心包片成形技术预防瓣周漏发生。方法 2016年1月-2017年2月,对33例容易发生瓣周漏的病人,先使用自体心包片行瓣环成形术,再行瓣膜替换手术。其中感染性心内膜炎22例,白塞氏病2例,瓣膜退行性变2例,瓣环严重钙化8例。根据不同的解剖特点选择不同的成形技术。结果 33例瓣膜替换手术病人术后及2年复查均无瓣周漏发生,无晚期死亡,无其他心血管事件。结论使用自体心包片成形技术,能有效预防瓣膜替换手术中瓣周漏的发生。

【Abstract】 Objective To discuss the causes of perivalvular leakage in valve replacement surgery, anatomic factors and the use of autologous pericardial sheet forming technology to prevent the occurrence of perivalvular leakage. Methods The 33 patients prone to perivalvular leakage after valve replacement, were undergoing autologous pericardial disc annuloplasty before valve replacement surgery. There were 22 cases with infective endocarditis, Behcet’s disease in 2 cases, valvular degeneration in 2 cases, disc ring severe calcification in 8 cases. According to different anatomical characteristics we choose different forming technology. Results None of the patients had a perivalvular leakage, no terminal death, no other cardiovascular events. Conclusion Using autologous pericardial sheet forming technology, it can effectively prevent perivalvular leakage after valve replacement operation.
