炫速双能CT、肺灌注成像及下肢静脉联合成像技术对静脉栓塞性疾病早期诊断的临床研究

【摘要】 目的该文主要讨论炫速双能CT(DE-CTPA)、肺灌注成像(DE-PI)及下肢静脉联合成像技术(下肢CTV)对静脉栓塞性疾病(venous thromboembolism,VTE)早期诊断的临床研究情况。方法选取2017年1—6月间于该院治疗的高度疑似肺栓塞(pulmonary embolism,PE)患者共20例,均行常规DSA法诊断联合DE-CTPA、DE-PI联合下肢CTV技术进行诊断,对比观察两种方法的诊断效果。DE-CTPA、DE-PI联合下肢CTV以下记为CT组;DSA法记为DSA组。结果 DSA金标准检查共得出PE 5例、下肢深静脉血栓(deep venous thrombosis,DVT)9例。CT组检出盆腔DVT 8例,下肢DVT 10例,DSA组检出盆腔DVT 5例,下肢DVT10例,两组在盆腔DVT检查中差异有统计学意义(χ2=4.02,P=0.019);下肢DVT检查中差异无统计学意义(χ2=2.02,P=0.067)。结论 DE-CTPA、DE-PI联合下肢CTV技术具有成像速度快、图像质量高,且具有无创性和多视角观察的优点,值得临床推广使用。

【Abstract】 Objective This paper tries to discuss the clinical research in early diagnosis of venous thrombotic disease with DE-CTPA, DE-PI and lower limb CTV. Methods 20 patients with pulmonary embolism(PE) treated in this hospital from January to June 2017 were selected in this study. Routine DSA was used to diagnose combined with DE-CTPA and DE-PI combined with lower limb CTV diagnosis, and diagnosis effect of the two methods were contrasted, DE-CTPA, DE-PI combined with lower limb CTV were regarded as the CT group; DSA method was regarded as the DSA group. Results A total of 5 cases of PE were obtained by DSA gold standard examination, 9 cases of deep venous thrombosis,(DVT). There were 8 cases of pelvic DVT, 10 cases of lower extremity DVT, 5 cases of pelvic DVT and 10 cases of lower extremity DVT. There were significant differences between the two groups in pelvic DVT, and the difference was statistically significant(χ2=4.02, P=0.019); there was no difference in DVT examination(χ2=2.02, P=0.067).Conclusion DE-CTPA, DE-PI combined with lower limb CTV technology has the advantages of fast imaging speed,high image quality and noninvasive and multi-angle observation, and it is worthy to be popularized.
