血清NGAL与慢性肾病患者冠状动脉斑块钙化相关性研究
The Association between Serum Neutrophil Gelatinase-associated Lipocalin and Coronary Atherosclerosis Calcification in Patients with Chronic Kidney Diseases
【摘要】 目的探讨慢性肾病患者中性粒细胞明胶酶相关载脂蛋白(NGAL)与冠状动脉粥样硬化之间的相关性。方法连续收集2013年3月至2015年12月在吴忠市人民医院住院行冠状动脉计算机断层扫描血管造影(CTA)检测的各型慢性肾病患者138例,ELISA检测患者血清NGAL水平,冠状动脉CTA分析患者冠状动脉粥样硬化的分布和影像学特点,单因素和多因素回归分析冠状动脉粥样硬化的独立影响因素,Peasrson和Sperman相关分析探究血清NGAL水平与冠状动脉病变特点的相关性。结果冠状动脉粥样硬化组患者血清NGAL水平高于冠状动脉正常组(P=0.001)。NGAL与hs-CRP、斑块数目、病变血管节段数目和钙化斑块呈正相关,与eGFR呈负相关(P<0.05);校正后血清NGAL水平是慢性肾病患者冠状动脉粥样硬化的独立预测因子(P=0.024,OR=1.003,95%CI:1.001~1.004)。结论血清NGAL是慢性肾病患者发生冠状动脉粥样硬化的重要炎症标志物,在评估慢性肾病患者冠心病风险中具有重要价值。
【Abstract】 Objective To explore the relationship between serum neutrophil gelatinase associated lipocalin(NGAL)and coronary atherosclerosis calcification in patients with chronic kidney diseases. Methods A total of 138(2013,March-2015,Decemeber)consecutive patients with chronic kidney diseases were enrolled in this observational study. NGAL were detected by ELISA kit. The morphology and distribution of coronary lesion were evaluated by coronary CT angiography. Univariate and multivariate logistic regression analyses were performed to determine the independent predictors of coronary atherosclerosis. Pearson and Spearman correlation analysis were conducted to determine the relationship between NGAL and coronary lesion morphology. Results For chronic kidney diseases patients,NGAL level was significantly increased in the CAD group than that in the non-CAD group(P=0.001). sST-2 levels were correlated positively with high-sensitivity C-reactive protein,the number of plaques and the calcified plaque type(P<0.05). NGAL were a significant independent predictor of coronary atherosclerosis in chronic kidney diseases patients(P=0.024;odds ratio=1.003;95% confidence interval:1.001-1.004). Conclusion NGAL is a promising inflammation biomarker that may help identify coronary atherosclerosis in patients in patients with chronic kidney diseases.
