不同骨材料对上颌窦底提升术骨吸收和种植体存留的影响

【摘要】 目的探讨不同骨材料对上颌窦底提升术骨吸收率和种植体存留率的影响。方法选择医院收治的48例牙列缺损患者,全口曲面体层X线片检查显示上颌后部分剩余牙槽骨高度≤4 mm,均接受经上颌骨前外侧壁的上颌窦底提升术和植骨延期种植术治疗,根据骨移植材料的不同,将患者分为2组,其中28例采用单纯异种骨移植材料作为异种骨组,20例采用自体骨联合异种骨移植材料作为混合组。结合X线片随访结果,比较两组植骨术后骨吸收率和种植体存留率。结果 48例均成功完成上颌窦底提升术和延期种植体植入,术后随访2年,两组移植骨量均有一定程度减少,但异种骨组随访期末植骨面积(45.75±6.91)mm~2大于混合组(40.04±4.57)mm~2,骨吸收率(1.99%)低于混合组(15.24%)(P<0.05);异种骨组、混合组分别植入57、42枚种植体,随访存留率分别为98.25%、97.62%,二者无统计学差异(P>0.05)。结论上颌窦底提升术采用异种骨移植材料的骨吸收率低于异种骨联合自体骨,但二者对延期种植体存留效果无明显影响。

【Abstract】 Objective To explore the effects of different bone materials on bone resorption and implant retention rates in maxillary sinus floor lifting. Methods A total of 48 patients with dentition defect to receive treatment in our department were selected.Panoramic tomography X-ray examination showed that the residual alveolar bone height at the maxillary posterior part ≤4 mm. All patients were treated with maxillary sinus floor lifting and delayed implantation of bone graft on the maxillary anterior lateral wall.These patients were divided into an xenogeneic bone group(n=28, treated with simple xenogeneic bone graft material), and a mixed group(n=20, treated with autogenous bone combined with xenogeneic bone graft material. The bone resorption rate and retention rate between the two groups after bone grafting were compared according to the results of the X-ray follow-up visit. Results Both maxillary sinus floor lifting and delayed implantation were successfully conducted on 48 patients. During the two-year follow-up visit,the amount of bone graft in the two groups decreased to some extent, but the bone graft area(45.75 ±6.91)mm~2in the xenogeneic bone group at the end of the follow-up visit was greater than that(40.04 ±4.57) mm~2 in the mixed group, and the bone resorption rate(1.99%) was lower than that(15.24%) in the mixed group(P < 0.05). 57 and 42 implants were respectively implanted into the xenogeneic bone group and the mixed group; the retention rate during the follow-up visit was 98.25% and 97.62% respectively,showing no significant difference(P > 0.05). Conclusion The bone absorption rate of maxillary sinus floor lifting with xenogeneic bone graft material is lower than that of xenogeneic bone combined with autologous bone, but the two methods have no significant impacts on the retention of delayed implant.
