【摘要】 目的观测中国成人肩袖韧带止点的解剖学形态,为临床肩袖修补手术提供基础理论指导。方法取10具防腐处理成人尸体标本,供体男性7具,女性3具。年龄22~46岁,平均30.2岁。左肩10侧,右肩10侧。采取游离肱骨远端,制作标本并观察标本形态,然后对冈上肌肌腱、冈下肌肌腱、小圆肌肌腱、肩胛下肌肌腱实质部及肱骨大结节止点进行解剖测量。结果标本冈上肌肌腱平均宽度为(25.18±3.47)mm,平均厚度为(2.61±1.81)mm,在肱骨大节结止点面积为(156.28±22.52)mm~2;冈下肌肌腱平均宽度为(25.48±4.14)mm,平均厚度为(3.06±0.75)mm,在肱骨大节结止点面积为(143.87±39.37)mm~2;小圆肌肌腱平均宽度为(14.68±3.16)mm,平均厚度为(2.32±0.88)mm,在肱骨大节结止点面积为(98.95±18.73)mm~2;肩胛下肌肌腱平均宽度为(24.45±5.63)mm,平均厚度为(4.25±1.31)mm,在肱骨大节结止点面积为(123.64±26.58)mm~2;冈上肌肌腱与冈下肌肌腱在肱骨大节结止点处有部分重叠,重叠处的平均宽度为(19.33±5.58)mm,在肱骨大节结止点重叠面积为(43.04±6.64)mm~2。结论构成肩袖的4块肩袖肌以一定的顺序包绕着肩关节,冈上肌肌腱、冈下肌肌腱及小圆肌肌腱附着于肱骨大结节的上部、中部及下部依次排列,肩胛下肌附着于小结节上,且冈下肌肌腱在肱骨大节结止点处有部分腱性部分覆盖在冈上肌之上形成部分重叠,在进行肩袖修补或重建时,需考虑其解剖学特点。更多还原

【Abstract】 Objective To observe the anatomical morphology of the rotator cuff ligament attachment of Chinese adults,which can provide theoretical guidance for the clinical rotator cuff repair surgery.Methods 10 embalmed adult specimens were ennrolled,including 7males and 3females.The specimens’ age ranged from 22 to 46years old,and the average age was 30.2years old.We also took 10 side of the left shoulder and 10 side of the right side.Collecting the free distal humerus and observing the morphological of specimens,then we dissected and measured the supraspinatus muscle tendon,the infraspinatus muscle tendon,the knolls muscle tendon,the substantial part of lower-scapular muscle tendon and the attachment of the greater tubercle humerus.Results The average width of the supraspinatus muscle tendon of the specimens was(25.18±3.47)mm,and its average thickness was(2.61±1.81)mm,and the area of it in the attachment of the greater tubercle humerus was(156.28 ± 22.52)mm~2.The average width of the infraspinatus muscle tendon of the specimens was(25.48±4.14)mm,and the average thickness of it was(3.06±0.75)mm,and the area of it in the attachment of the greater tubercle humerus was(143.87±39.37)mm~2.The average width of the knolls muscle tendon was(14.68±3.16)mm,and the average thickness of it was(2.32±0.88)mm,and the area of it in the attachment of the greater tubercle humerus was(98.95±18.73)mm~2.The average width of the lower-scapular muscle tendon was(24.45±5.63)mm,and its average thickness was(4.25±1.31)mm,and its area in the attachment of the greater tubercle humerus was(123.64±26.58)mm~2.There were partially overlap between the supraspinatus muscle tendon and the infraspinatus muscle tendon in the attachment of the greater tubercle humerus.The average width of overlap was(19.33±5.58)mm,and its area was(43.04±6.64)mm~2.Conclusion The supraspinatus muscle tendon,the infraspinatus muscle tendon and the knolls muscle tendon were lined up in the greater tubercle humerus trochiter with the top part,the middle part and the bottom.The lower-scapular muscle was attached to the lesser tubercle.Partially overlapped between the supraspinatus muscle tendon and the infraspinatus muscle tendon,and we need to consider the characteristics of anatomy in rotator cuff repair or reconstruction.更多还原
