QRS波群宽度与老年非ST段抬高型急性冠状动脉综合征患者住院期间主要心脏不良事件的关系

【摘要】 目的探讨老年非ST段抬高型急性冠状动脉综合征(NSTE-ACS)患者入院时心电图QRS波群宽度(QRSw)与主要心脏不良事件(MACE)的关系。方法将137例老年NSTE-ACS患者按照入院时心电图QRSw分为QRSw≤100ms组和QRSw>100ms组。同时,按照患者住院期间是否发生MACE分为MACE组和非MACE组,并利用Logistic回归方程分析QRSw与MACE发生的关系。结果 QRSw≤100ms组95例,QRSw>100ms组42例,QRSw>100ms组血清c Tn I、CK-MB、hs-CRP、BNP水平及MACE发生率均高于QRSw≤100ms组,LVEF低于QRSw≤100ms组,差异均有统计学意义(均P<0.05)。MACE组31例,非MACE组106例。MACE组QRSw大于非MACE组,血清c Tn I、CK-MB、hs-CRP、BNP水平高于非MACE组,差异均有统计学意义(均P<0.05)。Logistic回归分析显示QRSw延长是老年NSTE-ACS患者住院期间发生MACE的危险因素(OR=1.213,95%CI:1.103~1.435,P<0.05)。结论 QRSw延长老年NSTE-ACS患者住院期间发生MACED的危险性增加。

【Abstract】 Objective To investigate the association of QRS wave width(QRSw) with in-hospital major adverse cardiac events(MACE) of elder patients with non-ST segment elevation acute coronary syndrome(NSTE-ACS). Methods137 elder patients with NSTE-ACS were divided into QRSw ≤100 ms group(group A, n=95)and QRSw>100 ms group(group B, n=42)based on ECG recorded at admission. Logistic regression analysis was performed for the relationship between QRSw and MACE. Results Serum c Tn I, CK-MB, hs-CRP and BNP levels were significantly higher and LVEF significantly lower in group B than in group A(all P<0.05). QRS was significantly wider, serum c Tn I, CK-MB, hs-CRP and BNP levels were significantly higher in 31 patients with MACE than in106 patients without MACE(all P <0.05). Logistic regression analysis showed that the increase of QRSwwas an independent risk factor of in-hospital MACE in elder patients with NSTE-ACS(OR = 1.213, 95%CI: 1.103~1.435, P<0.05). Conclusion The risk of in-hospital MACE increases in elder patients with NSTE-ACS and wide QRS.
