【摘要】 目的:应用实时三维超声心动图(RT-3DE)与组织多普勒方法测量右心室Tei指数相结合,评价系统性红斑狼疮患者的右心结构和功能。方法:对39名SLE患者和13例健康者采集右心常规超声图像和三维超声动态图像并存储,用组织多普勒方法计算出Tei指数(TDI-Tei),使用RT-3DE测得右心室舒张末期和收缩末期容积及右心室的射血分数(RVEF)。结果:RVEF-3D显示中-重度PAH与对照组、SLE无合并PAH、轻度PAH组比较,右心室舒张末期容积、右心室收缩末期容积明显增大,右心室射血分数明显减低(P<0.05),轻度PAH组与SLE无合并PAH及对照组数据无明显差异(P>0.05);不同组间的TDI-Tei数值差异较大(P<0.05)。结论:SLE患者早期即可出现RV舒张功能降低,逐渐收缩功能减退越明显。RT-3DE能够准确地评估右心室的收缩功能;若结合TDI-Tei,可早期评估右心室整体功能,这对于SLE患者心功能的评估具有重要意义。更多还原

【Abstract】 Objective: To assess the right heart function and cardiac structure in patients with systemic lupus erythematous( SLE) using real-time three dimensional echocardiography( RT-3DE) combined with right ventricular Tei index measurement. Methods: The images of conventional echocardiography and RT-3DE were obtained in 39 SLE patients and 13 healthy volunteers.Right ventricular Tei index( TDI-Tei) and right ventricular ejection fraction( RVEDV) measured at the end diastolic and systolic phase were calculated.Results: Distended volume at the end diastolic and systolic phase was observed in patients with medium to severe pulmonary arterial hypertension( PAH) compared to controls and SLE patients without PAH and those with mild PAH,whereas RVEDV was significantly decreased( P<0.05).The difference was not significant in patients with mild PAH and SLE patients without complicated PAH( P>0.05).TDI-Tei varied a lot in patients in different groups( P<0.05).Conclusion: Decreased right ventricular function may be seen in the early stage in patients with SLE,and RT-3DE can exactly reveal the right heart function at systolic phase.However,combined RT-3DE with TDI-Tei can demonstrate full function of the right heart in early stage for patients.更多还原
