2012-2014年临床分离金黄色葡萄球菌的感染分布及耐药性分析
Infection Distribution and Antibiotic Resistance Analysis for Clinical Isolates of Staphylococcus Aureus in 2012-2014
【摘要】 目的回顾性分析2012年至2014年我院各种临床标本分离的金黄色葡萄球菌的药敏情况,为临床合理使用抗生素提供依据。方法临床标本分离培养,按照《全国临床检验操作规程》(第3版)进行操作,用全自动细菌鉴定仪Vitek-2 compact进行菌种鉴定及药敏试验,以K-B法测替考拉宁的药敏,使用WHONE5.6版软件进行统计学分析。结果 3年内共分离了金黄色葡萄球菌759株,标本主要来自痰液,占43.48%,其次是创面分泌物,占16.00%,检出率前三位的科室是脑外科14.22%、ICU 13.44%、门诊13.44%,其中耐甲氧西林金黄色葡萄球菌(MRSA)为388株,其中2012年MRSA检出率为53.84%,2013年为43.36%,2014年为54.95%。调查表明18种测试药物的耐药率在3年期间并无明显变化。所有菌株对青霉素耐药最严重,为96.05%,其次是红霉素,为67.19%。对替考拉宁、万古霉素、利奈唑胺、奎奴普丁/达福普汀均100%敏感,对其余的抗菌药物耐药率在17.92%-54.94%,可见金黄色葡萄球菌多重耐药现象比较严重。结论耐甲氧西林金黄色葡萄球菌(MRSA)的耐药性高于甲氧西林敏感的金黄色葡萄球菌(MSSA),因此,加强对MRSA的耐药性监测,对指导临床合理用药具有非常重要的意义。
【Abstract】 Objective To analyze drug sensitivity of Staphylococcus aureus isolated of various clinical specimens in our hospital in 2012 to 2014, provide the basis for rational use antibiotics in clinical. Methods In clinical specimens isolated and cultured in accordance with "National Clinical Laboratory Operating Procedures"(3 rd Edition) operating with automatic bacterial identification system Vitek-2 compact strain identification and susceptibility testing to K-B method to measure the susceptibility of teicoplanin using WHONE 5.6 version software for statistical analysis. Results A total of 759 strains of Staphylococcus aureus were isolated in 3 years, mainly from sputum specimens, accounting for 43.48%, followed by wound secretions, accounting for 16.00%, the detection rate of the top three departments neurosurgery is 14.22%, ICU13.44%, outpatient 13.44%, methicillin resistant Staphylococcus aureus(MRSA) is 388, which in 2012 MRSA detection rate of 53.84%, in 2013 to 43.36%, in 2014 54.95%. The resistance rates of the 18 tested drugs did not change significantly during the 3 year period. All the most serious strains resistant to penicillin, 96.05%, followed by erythromycin, 67.19% of the tensile test-taking Ning, vancomycin, linezolid, quinupristin/dalfopristin were 100% sensitive to the rest of the antimicrobial resistance rates of between 17.92%-54.94%, showing that the phenomenon of multi-drug resistant Staphylococcus aureus comparison serious. Conclusions Methicillin-resistant Staphylococcus aureus(MRSA) resistance higher than methicillin-sensitive Staphylococcus aureus(MSSA), therefore, strengthen the monitoring of MRSA resistance to guide clinical use of drugs has very important.
