颅内压、脑温在重型颅脑损伤中的相关性研究
The Correlation Study Between Intracranial Pressure and Brain Temperature in Severe Head Injury
【摘要】 目的探讨重型颅脑损伤后颅内压(intracranial pressure,ICP)、脑温与预后的关系。方法对70例重型颅脑损伤患者(其中62例手术治疗,8例保守治疗)进行不间断的ICP、脑温监测,并分析二者之间的关系。结果 3月后随访,14例预后良好(20.00%),轻残16例(22.86%),重残17例(24.29%),植物生存4例(5.71%),19例死亡(27.14%)。死亡病例的脑温逐渐降低,ICP值逐渐升高,与存活组的相应值比较具有显著性差异(P<0.01)。结论重型颅脑损伤患者的ICP、脑温之间在一定范围内存在相关性。预后良好组ICP、脑温没有相关性;残疾组ICP、脑温有相关性;死亡组ICP、脑温在ICP高于40mm Hg、脑温低于37℃存在显著的负相关性。但重型颅脑损伤后ICP、脑温监测对预后评估及临床治疗有一定预见性。
【Abstract】 Objective To study the relationship among ICP(intracranial pressure) and BT(brain temperature) and the recovery in severe head injury patients. Methods Continuous monitoring the ICP and BT were tested in 70 severe head injury patients(62 cases of surgical treatment, and 8 of conservative treatment), and the relationship was analyzed among them. Result The results were as follows by follow-up 3 months later: 14(20.00%) cases with good recovery, 16(22.86%) mild disability, 17(24.29%) severe disability, 4(5.71%) persistent vegetative state and 19(27.14%) death. The BT reduced gradually, while the ICP increased in the death cases. Conclusion In patients with severe traumatic brain injury between ICP,BT in a range of memory in correlation. Good prognosis group ICP, BTno correlation; disability group ICP, BTwere correlated; death group ICP, BTis higher than the ICP 40 mm Hg, BTis below 37℃ there was a significant negative correlation. However, after severe traumatic brain injury ICP, BT monitoring of prognosis and clinical treatment has some predictability.
