P16、P53在宫颈高级别鳞状上皮内病变中的表达及临床意义

【摘要】 目的通过观察在HSIL中P16、P53的表达情况,说明P16、P53作为标记物在病变组织的意义。方法选取2009年8月至2015年8月在中国医科大学附属第一医院治疗的宫颈病变病人进行研究,根据组织学诊断结果,将其分成两个亚组,CINII级120例,记为观察A组;CINIII级56例,记为观察B组;另选取40例非宫颈病变病人作为对照组。采用免疫组化SP法,检测P16、P53蛋白的表达。结果 (1)P16在对照组,宫颈CINII和CINIII中的阳性表达率差异具有统计学意义(P<0.05),P16表达逐渐增强,经Spearman相关性分析,相关系数0.525(P<0.05)。(2)P53在对照组,宫颈CINII和CINIII中的阳性表达率差异具有统计学意义(P<0.05),P53表达逐渐增强,经Spearman相关性分析,相关系数0.500(P<0.05)。结论 P16、P53在HSIL表达阳性率明显高于正常健康人群,随CIN级别升高,P16、P53阳性表达显著升高,两者与CIN临床分级存在正向相关性。

【Abstract】 Objective to observe expression of P16 and P53 in HSIL, and illustrate significance of P16 and P53 as markers in diseased tissues. Methods choose cervical lesions patients treated in China Medical University the First Affiliated Hospital from August 2009 to August 2015 for study, according to histological diagnosis, divide them into two subgroups, 120 cases grade CINII was recorded as group A; 56 cases grade CINIII as group B; choose another 40 cases non-cervical lesions patients as control group. With immunohistochemical SP method, detect expression of P16 and P53 protein. Results(1) positive expression rate difference of P16 in control group, cervical CINII and CINIII showed statistical significance(P< 0.05), P16’s expression increased gradually with correlation coefficient of 0.525 by Spearman correlation analysis(P< 0.05).(2) positive expression rate difference of P53 in control group, cervical CINII and CINIII showed statistical significance(P< 0.05), and expression of P53 increased gradually with correlation coefficient of 0.500 by Spearman correlation analysis(P< 0.05). Conclusion positive expression rate of P16 and P53 in HSIL was higher than normal healthy people, and with increase of CIN level, positive expression rate of P16 and P53 increased significantly, which has positive correlation with clinical level of CIN.
