【摘要】 目的:研究白细胞介素23（IL-23）对强直性脊柱炎（AS）患者的临床诊断意义及其受体表达情况。方法:选择2012年11月至2013年11月丹江口市妇幼保健院收治的AS患者40例为AS组,另外选择同期健康体检者40例作为健康对照组。测定两组受试者血清IL-23、红细胞沉降率（ESR）、C反应蛋白（CRP）和免疫球蛋白（IgA）水平;收集分离两组受试者外周血中单个核细胞（PBMCs）并行体外细胞培养,收集细胞,利用Western blotting技术检测IL-23 p19R蛋白表达变化。结果:AS患者血清中IL-23水平显著高于健康对照组[（630±69）ng·L^-1vs（84±7）ng·L^-1,P<0.05],且AS组血清IL-23水平与Bath AS疾病活动指数（BASDAI）、ESR和CRP的浓度呈正相关（r值分别为0.598、0.602、0.548、0.436,P值分别为0.004、0.005、0.040、0.030）。AS组PBMCs经IL-23刺激后,其IL-23 p19R蛋白表达显著升高。结论:AS患者血清中IL-23水平与BASDAI、ESR、CRP及IgA的水平变化呈正相关,IL-23可能通过改变IL-23 p19R蛋白的变化来介导CD₄⁺T向Th17分化,参与AS的发病。更多还原

【Abstract】 Objective: To study the significance of IL-23 on the diagnosis of ankylosing spondylitis（ AS） and its receptor expression. Methods: The serum level of IL-23 in AS patients and healthy controls,erythrocyte sedimentation rate（ ESR）,C-reactive protein（ CRP） and immunoglobulin（ IgA） were detected; the peripheral blood mononuclear cells（ PBMCs） were separated and collected in the AS patients and healthy controls,the cells were then cultured in vitro,IL-23 p19 R protein expression was assessed using Western blotting technique. Results: The concentration of serum IL-23 in AS patients significantly increased compared with healthy controls group with a significant difference [（ 630 ± 69） ng·L-1vs（ 84 ± 7） ng · L^-1,P < 0. 05],and the concentration of serum IL-23 with BASDAI index,ESR and CRP concentrations were positively correlated（ r value were 0. 598,0. 602,0. 548,0. 436,P values were 0. 004,0. 005,0. 040,0. 030,respectively）.
