【摘要】 目的:分析大肠侧向发育型肿瘤(LST)内镜下综合诊治临床疗效及体会。方法:收集2015年8月至2017年1月在东莞市人民医院治疗的69例LST患者资料,根据患者肿瘤情况给予其内镜黏膜切除术(EMR)、黏膜剥离术(ESD)、分片切除术(EPMR)治疗,分析3种手术治疗效果。结果:超声内镜(EUS)检出混合结节型17例、颗粒均一型27例、扁平隆起25例;病灶直径为10~50 mm,分布于黏膜层49例、下层黏膜受累20例;手术:外科手术7例、EMR 20例、ESD 19例、EPMR 23例,切除完整率76.81%,并发症率13.04%,复发率10.13%。结论:以内镜下EMR、ESD、EPMR术式治疗LST疾病均有良好效果,其中,ESD病灶清除较彻底,复发率低,应用价值良好。更多还原

【Abstract】 Objective To analyze the clinical effi cacy and experience of comprehensive treatment of colorectal lateral spreading tumor(LST). Methods The clinical data of 69 patients with LST treated in our hospital from August 2015 to January 2017, the patient were performed by endoscopic mucosal resection(EMR), endoscopic submucosal dissection(ESD), endoscopy piecemeal mucosal resection(EPMR) treatment according to its tumor condition. Three kinds of surgical treatment were analyzed. ResultsBy detection with EUS was found 17 cases of mixed nodules, granules were 27 cases, fl at uplift in 25 cases; Lesion diameter of 10-50 mm, distributed in the mucous membrane was 49 cases, and 20 cases of lower mucosal involvement; Surgery: 7 cases of surgery, EMR with 20 cases, ESD of 19 cases, EPMR has 23 cases, the removal rate was 76.81%, the complication rate was 13.04 %, and the recurrence rate was 10.13 %. Conclusion The patients underwent endoscopic EMR, ESD and EPMR to treat LST diseases has a good clinical results, among them, ESD lesions more thorough removal, the recurrence rate was low, the application value was good.更多还原
