【摘要】 目的比较Shikani可视喉镜和Clarus可视喉镜(Trachway)在颈椎制动患者气管插管中的应用效果。方法选择ASAⅠ~Ⅱ级,拟全身麻醉下行颈椎内固定术的患者60例。按随机数字表法分为Shikani可视喉镜(S组)和Clarus可视喉镜组(C组),每组30例。记录麻醉诱导前(T1)、麻醉诱导后插管前(T2)、插管后即刻(T3)、插管后1 min(T4)、3 min(T5)、5 min(T6)时刻的平均动脉压(MAP)、心率(HR)、心率-收缩压乘积(RPP)的情况,记录插管时间、1次插管成功率、插管次数、咽喉疼痛及相关并发症发生情况。结果 S组一次气管插管完成时间明显短于C组(P<0.05),轻度咽喉疼痛发生率及插管咽喉损伤率S组小于C组(P<0.05)。与T1比较,两组T2~T6时MAP及RPP降低(P<0.05),各时间点MAP、HR、RPP组间比较差异没有统计学意义(P>0.05)。首次插管成功率、插管次数两组比较差异没有统计学意义(P>0.05)。结论在颈椎制动患者气管插管中,与Clarus可视喉镜(Trachway)比较,Shikani喉镜插管时间缩短,但在插管时对血流动力学影响、首次插管成功率、插管次数及咽喉疼痛等相关并发症发生率等方面无明显差异。更多还原

【Abstract】 Objective To compare the application of Shikani optical stylet(SOS)and Clarus Video Stylet(Tracway)in patient with cervical spine immobilization in tracheal intubation. Methods Sixty patients,ASA Ⅰ~ Ⅱ,undergoing cervical internal fixation operation,were randomly divided into Shikani optical stylet group(Group S,n = 30)and Clarus Video Stylet group(Group C,n = 30). MAP,HR and RPP(the rate-pressureproduct)were recorded at the point before induction of anesthesia(T1),before intubation(T2),at the immediatetime of intubation(T3)and 1 min(T4),3 mins(T5),and 5 mins after intubation(T6). The intubation time,one-time success rate of intubation,the number of intubation times and the incidence of sore throat and other complica-tions were observed. Results The one-time intubation time in group S was obviously shorter than that in group C(P < 0.05). The incidence of mild sore throat and intubation throat injury rate were lower in group S than those ingroup C(P < 0.05). Compared with those at T1,MAP and RPP decreased significantly at point of T2~T6 in both oftwo groups(P < 0.05). There was no significantly difference in MAP,HR and RPP at any points of time betweenthe two groups. The one-time success rate of intubation,the number of intubation times and the incidence of airwaycomplications in two groups(P > 0.05)were no significantly different. Conclusions Compared with Clarus VideoStylet(Tracway),Shikani optical stylet can shorten the intubation time in patients with cervical spine immobiliza-tion,but no difference was found in regard to the hemodynamic influence on intubation,success rate of intubation,the intubation times,the sore throat and the other related complications.更多还原
