HBV前C区G1896A和G1899A促进e抗原血清学转换

【摘要】 目的:探讨乙型肝炎病毒(HBV)前C区1896位点和1899位点突变与e抗原血清转换和疾病进展的相关性。方法:收集HBs Ag阳性持续半年以上且血清HBV-DNA拷贝数>5.0×102IU/m L血清样本共238份,测序法分析HBV前C区1896和1899位点的碱基序列。同时收集样本的相关临床资料和HBe Ag的表达情况,利用SPSS 20.0,进行Spearman相关性分析和卡方检验。结果:HBV前C区1896和1899位点均可发生突变,由碱基G变成A,且均与e抗原表达密切相关(P<0.05)。G1896A和G1899A均能促进e抗原血清学转换,G1899A的转化率高于G1896A的转化率。G1899A与HBV相关疾病进展相关(相关系数0.280,P<0.05),尤其是HCC的发生。结论:HBV前C区G1896A和G1899A促进HBe Ag血清学转换。

【Abstract】 Objective To investigate the correlation of the 1896 and 1899 mutations of hepatitis B virus(HBV)with the conversion of e antigen in serum and the progression of the disease. Methods 238 serum samplesfrom the patients with HBs Ag positive for over six months and HBV-DNA copy number > 5.0 × 102IU/m L werecollected,and the sequence analysis was used to analyze the nucleotide sequences of the 1896 and 1899 sites inthe pre-C region of HBV. At the same time,the relevant clinical data and the expressions of HBe Ag werecollected,followed by Spearman correlation analysis and chi square test with SPSS 20.0. Results Both 1896 and1899 sites in the pre-C region of HBV were mutated,and the base G was A,which was closely related to theexpression of e antigen(P < 0.05). Both G1896 A and G1899 A promoted the e antigen serological conversion,andthe e antigen serological conversion of G1899 A was higher than that of G1896 A. G1899 A was associated with HBVrelated disease progression(correlation coefficient 0.280,P < 0.05),especially with the incurrence of HCC.Conclusions G1896 A and G1899 A in the pre-C region of HBV can promote the serological conversion of e antigen.
