七氟烷吸入对肺动脉高压大鼠右心功能影响的研究

【摘要】 目的 应用腹腔单次注射野百合碱建立大鼠肺动脉高压模型,观察及比较七氟烷吸入对肺高压大鼠及正常大鼠血流动力学的影响差异。方法 雄性SD大鼠30只随机分入肺高压组(M组)及对照组(C组),M组腹腔单次注射野百合碱60 mg/Kg。饲养28天后,两组大鼠分别进行开胸手术,置入右心PV导管。C组及M组分别梯度吸入0. 5%,1%,1. 5%浓度的七氟烷,观察七氟烷吸入对大鼠血流动力学的影响及对两组大鼠血流动力学影响的区别。结果 M组大鼠右室厚度及重量明显高于C组,但两组大鼠心输出量及射血分数无明显差异。七氟烷吸入使两组大鼠ESP、HR、MAP均降低,但M组大鼠每搏量、心输出量及心率指标优于C组大鼠,能更好的耐受七氟烷吸入。结论 低浓度七氟烷梯度吸入抑制肺高压大鼠和正常大鼠循环功能,但肺高压大鼠较正常大鼠更能耐受低浓度七氟烷吸入。

【Abstract】 Objective Intraoperative management of pulmonary hypertension represent a tremendous clinical challenge for the anesthesiologist. The effect of sevoflurane on PAH still need to be investigated. Methods Rats were randomly assigned to a control or a monocrotaline group( 60 mg kg ·1). 4 weeks after monocrotaline injection,animals were anesthetized,instrumented with an RV conductance catheter. Subgroups treated with sevoflurane underwent well-controlled dose-responses to 0. 5%,1%,and 1. 5%sevoflurane inhalation. The changes of hemodynamics were analyzed. Results Compared with controls,rats injected with monocrotaline presented RV hypertrophy,without changes in cardiac output and ejection fraction. The ESP,heart rate,MAP of both PAH and control rats decreased after sevoflurane inhalation,and PAH rats preserved more cardiac output,stroke volume,and heart rate. Conclusions In both PAH and normal rats,the right ventricular function was suppressed by sevoflurane,but PAH rats were more tolerant to low-dose sevoflurane inhalation.
