婴幼儿低剂量骨盆数字化X线摄影探讨

【摘要】 目的探讨在婴幼儿骨盆数字化X线摄影(DR)中运用低剂量摄影的可行性,及在自动曝光控制(automatic exposure control,AEC)下摄影管电压的选择对图像质量的影响,选取最佳照射条件。方法设置摄影管电压分别为60 kV,70 kV,80 kV三组,每组收集30例病例。使用自动曝光控制(AEC),曝光后分别记录曝光剂量m As,ms和剂量面积值(dose area product DAP)。获得90幅图像后,按照欧共体(CEC)图像质量标准对图像进行主观评价。运用SPSS 22.0进行统计学分析。结果 3组病例图像质量评价均符合诊断标准,不同kV组曝光剂量、曝光时间及剂量面积值均有统计学差异,随着kV值的增加,曝光剂量、曝光时间及剂量面积值逐渐减少,60 kV组剂量组曝光量值最高(4.10±0.71)mAs,(5.59±1.43)ms,(0.82±0.18)dGy·cm~2,其次是70 kV组(2.04±0.42)mAs,(3.30±0.74)ms,(0.60±0.17)dGy·cm~2,80 kV剂量组的曝光量值最低(1.10±0.34)mAs,(2.09±0.58)ms,(0.42±0.13)dGy·cm~2。根据骨盆诊断学要求的影像标准和重要影像细节标准对3组图像质量的主观评价结果显示3组间差异具有统计学意义,随着kV值的增加评分逐渐降低。结论 60 kV组图像质量最好,摄影剂量最高。80 kV组剂量最低,图像质量较其他2组差。自动曝光控制下70 kV组曝光剂量和图像质量最为平衡,70 kV管电压在婴幼儿骨盆数字化摄影中值得推荐。

【Abstract】 Objective To investigate the feasibility of using low dose photography in infant pelvic digital radiography, and the automatic exposure control(automatic exposure control AEC) under the influence of tube voltage se-lection on image quality, the best irradiation condition. Methods Set the tube voltage were 60 kV, 70 kV, 80 kV group of three cases were collected in each group of cases. The automatic exposure control(AEC) was used to record the exposure dose m As, ms and dose area value(dose area product DAP) after exposure, and 90 images were obtained. According to the European Union(CEC) image quality standards for the subjective evaluation of the image,using SPSS 22.0 for statistical analysis. Results The image quality evaluation of three groups of cases were consistent with the diagnostic criteria, different kV group mAs, ms, and dGycm~2 were statistically significant, with the increase of kV value, m As, ms, and d Gy·cm2 gradually decreased, 60 kV group dose exposure value up to MAS(4.10 ±0.71)mAs(5.59±1.43)ms,(0.82±0.18)dGy·cm~2, followed by 70 kV group(2.04±0.42)mAs,(3.30±0.74)ms,(0.60±0.17)dGy·cm~2, the lowest value of mAs exposure dose of 80 kV group(1.10 ±0.34) mAs(2.09 ±0.58)ms,(0.42 ±0.13)dGy·cm~2. According to the diagnostic criteria and important image details, the subjective evaluation of image quality of the three groups showed statistical difference between the three groups, with the kV value increased gradually decreased.Conclusion The 60 kV group had the best image quality and the highest dose. The 80 kV group had the lowest dose and the image quality was worse than the other two groups. The exposure dose and the image quality of the 70 kV group under the automatic exposure control were the most balanced, and the 70 kV tube voltage was recommended in the digital radiography of the pelvis.
