【摘要】 目的分析强直性脊柱炎的影像学表现,评价影像检查对强直性脊柱炎早期诊断价值。方法收集35例强直性脊柱炎患者影像资料,结合文献进行分析。结果 35例强直性脊柱炎患者全部侵犯骶髂关节,其中Ⅱ级10例,Ⅲ级18例,Ⅳ级7例;早期表现:骶髂关节面侵蚀、破坏,骨质局限性硬化,关节间隙正常;病变逐步进展,骶髂关节破坏加重,关节间隙变窄或增宽,部分呈骨性强直。其中28例患者骶髂关节破坏后,病变向上发展侵及脊柱,形成"方形椎"或"竹节状"脊柱,造成脊柱强直。强直性脊柱炎患者累及外周关节,最常累及髋关节,本组13例累及髋关节,表现为髋关节间隙对称性狭窄,关节面破坏。14例出现附丽病,表现为肌腱、韧带附着处骨化。结论影像学检查已成为诊断强直性脊柱炎的重要途径。更多还原

【Abstract】 Objective To analyze the imaging manifestations of ankylosing sondylitis, evaluate the value of imaging examination in the diagnosis for ankylosing spondylitis. Methods Thirty-five cases with AS were retrospectively analyzed by reviewing literature. Results The arthritics of sacroiliac joint were found in all 35 cases of which10 cases were defined by II grade, 18 cases were defined by III grade and 7 cases were defined by IV grade. In early stage, the characteristic of AS present with normal sacroiliac joint space, but the erosion, localized sclerosis of bone;in later stage, the joint space narrowing or widening, damage aggravated, partial ankylosis. Spine like bamboo and square vertebra were found in 28 cases with sacroiliac joint damage as spine abnormal was developed step by step. Hip joint abnormal often being involved in AS which were found in 13 cases, appeared as symmetrical narrowing of the hip joint space and damage of articular surface. Fourteen cases had enthesopathy that appeared as ossification of tendon and ligament attachment. Conclusion Imaging examination has become an important way to diagnose ankylosing spondylitis.更多还原
