【摘要】 目的 探讨游离移植股前外侧穿支皮瓣在修复四肢软组织大面积缺损的临床应用效果。方法2008年2月-2016年5月,选用股前外侧穿支皮瓣游离移植修复四肢皮肤软组织大面积缺损29例。3例伴有肢体主干血管损伤者同时给予皮瓣蒂部血管血流桥接,恢复肢体血运。术后随访测评皮瓣外形、感觉、血运和供区恢复情况等,对临床资料进行综合分析。结果 本组29例,除3例皮缘部分坏死,予植皮和换药Ⅱ期愈合外,其余26例Ⅰ期愈合。随访3~42个月,平均12.8个月,皮瓣外形满意,恢复保护性感觉,两点辨别觉为12~15 mm,质软有弹性。皮瓣供区外形恢复好,对下肢功能无明显影响。3例伴有肢体主干血管损伤者同时给予血管蒂部血流桥接,术后肢体血运恢复好。结论 游离移植股前外侧穿支皮瓣修复四肢软组织大面积缺损外形满意,皮瓣供区外形恢复好,对同时伴有肢体主干血管损伤者可以血流桥接,恢复肢体血运,是一种理想的手术方法。更多还原

【Abstract】 Objective To evaluate the free anterolateral thigh perforator flap in repairing large area defects of limb soft tissue. Methods From February 2008 to May 2016, 29 cases were freed anterolateral thigh perforator flap to repair soft tissue large area defects. The injury of main blood vessel in limbs was occured in 3 cases. Meanwhile,reconstructed the blood supply through blood flow bridge in pedicle vessel of flaps.The shape of the flap, the blood circulation and sensory were observed and researched. Results All of 29 cases, 26 cases survived; 3 cases of partial necrosis edge, healed with dressing and skin graft. The patients were followed up for 3 to 42 months postoperatively, 12.8 months in average. The flap had a good appearance and sensory. The two-point discrimination was about 12 to 15 mm. The flap was soft and elastic. The recovery of donor site was good. No damage to the function of lower extremity. The injury of main blood vessel in limbs was occured in 3 cases. Meanwhile, reconstructed the blood supply through blood flow bridge in pedicle vessel of flaps. The blood supply was good postoperation. Conclusion It is good in outline and recovery of blood supply of flap, that freeing anterolateral thigh perforator flap to repair large area defects of limb soft tissue.
