晚期恶性肿瘤患者生存时间与血清胆碱酯酶水平的相关性分析

【摘要】 目的探讨血清胆碱酯酶(CHE)与晚期肿瘤患者生存时间(ST)的相关性。方法选取2013年3月至2016年3月140例恶性肿瘤病例资料进行回顾性分析,统计入院时患者的10项指标,分别为:CHE、天门冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、前白蛋白(PA)、白蛋白(ALB)、甲胎蛋白(AFP)、癌胚抗原(CEA)、糖类抗原199(CA199)、纤维蛋白原(Fbg)、栓溶二聚体(Ddim)。以入院时患者上述生化、肿瘤标志物、凝血共10项指标为数据集,构建单因素线性回归预测模型(以ST为应变量,CHE值为自变量),采用enter法筛选变量,构建多重线性回归模型。结果男性CHE、CEA水平低于女性,两者比较差异具有显著性(P<0.05)。单因素线性回归预测模型结果:CHE与ST存在线性相关,CHE每升高100个单位,ST延长2.4d,ST=0.02×CHE-15.5,t=9.5,β=0.002,95%CI:0.02~0.03,P<0.05。多因素回归模型结果:CHE与消化道晚期肿瘤患者生存时间存在线性相关,差异具有显著性(P<0.05)。ALT、PA、ALB虽然随着三项指标数值的增大,患者ST延长,但差异无显著性(P>0.05)。女性ST低于男性,但差异无显著性(P>0.05)。Fbg随着其数值的增大患者ST缩短,差异具有显著性(P<0.05)。结论血清CHE水平可预测晚期肿瘤患者ST,与晚期肿瘤患者ST呈线性相关。

【Abstract】 Objective To discuss the correlation of survival time and serum cholinesterase level in terminal cancer atients. Methods From March 2013 to March 2016,140 cases of digestive tract malignant tumor cases data were retrospectively analyzed,and the statistics of 10 indicators of admission patients,including: CHE,aspartate amino transferase( AST),alanine aminotransferase( ALT) and prealbumin( PA),albumin( propagated),a tire protein( AFP),carcinoembryonic antigen( CEA) and carbohydrate antigen 199( CA199),fibrinogen( Fbg),pin stripping dimers( Ddim). A single factor linear regression prediction model was built( with ST as dependent variable,CHE values for the independent variables),using the enter method selection variables,and multiple linear regression model was constructed. Results CHE,CEA levels in male were lower than those of the female with significant difference( P < 0. 05). Single factor linear regression forecast model results: there was linear correlation of CHE with ST,CHE per increase 100 units,ST extended by 2. 4 days,ST = 0. 02 × CHE-15. 5,t = 9. 5,β = 0. 002,95% CI: 0. 02 ~ 0. 03,P < 0. 05). Multi-factor regression model results: CHE was associated with late digestive tract cancer patients survival time,with significant difference( P < 0. 05). ST was longer with the increase of ALT,PA,ALB,but there was no significant difference( P >0. 05). ST of female was lower than SE of man,but there was no significant difference( P > 0. 05). ST shortened when Fbg increased with significant difference( P < 0. 05). Conclusion CHE can predict ST,and it is linearly related with patients with ST of advanced tumor patients.
