【摘要】 目的对比退变性腰椎管狭窄症经两种融入方式的单节段经椎间孔腰椎间融合术治疗的疗效。方法选取80例退变性腰椎管狭窄症患者,随机分为观察组与对照组,各40例。对照组患者给予经后方入路椎体间孔融合术(PLIF),观察组患者给予切除上下关节突经椎间孔入路椎体间融合术(TLIF)。对两组患者的手术效果、手术相关指标、术后并发症及住院时间进行对比,并对比两组患者术前及术后视觉模拟评分(VAS)和Oswestry功能障碍指数(ODI)评分。结果观察组与对照组患者的手术优良率比较无显著差异,两组手术前与手术后的VAS评分、ODI评分比较无显著差异(P>0.05);观察组患者的手术时间与住院时间均较对照组明显缩短(P<0.05);观察组患者并发症总发生率为5.00%,对照组患者并发症总发生率为25.00%,观察组患者并发症发生率显著低于对照组(P<0.05)。结论 TLIF与PLIF治疗退变性腰椎管狭窄症均具有显著疗效,其中TLIF的围手术情况良好,且术后并发症低,临床应用价值更高。更多还原

【Abstract】 Objective To investigate the therapeutic effect of lumbar interbody fusion methods in patients with degenerative lumbar spinal stenosis. Methods 80 patients with degenerative lumbar spinal stenosis were selected,they were divided into observation group and control group,40 cases in each group. The control group were treated with posterior interbody fusion hole( PLIF),the observation group were treated by resection of articular transforaminal lumbar interbody fusion( TLIF),the effect of the operation of the two groups of patients,surgery related indicators,postoperative complications and hospitalization time were compared,and visual analogue scale( VAS) and Oswestry disability index( ODI)score of the two groups before surgery and postoperative were also compared. Results Operation excellent and good rate in observation group and control group,VAS score and ODI score before and after operation had no no significant difference,respectively( P > 0. 05). Operation time and hospitalization time of observation group were significantly shorter than those indexes of the control group( P < 0. 05). The overall complication rate in observation group was 5%,and in the control group 25%. The complication rate in the observation group was significantly lower than that of the control group( P < 0. 05). Conclusion TLIF and PLIF in the treatment of degenerative lumbar spinal stenosis has remarkable curative effect,the TLIF has good peri operation results and low postoperative complication rate,therefore,the clinical application value is higher.更多还原
