【摘要】 目的探讨弧形切割闭合器应用于中低位直肠癌保肛术的临床疗效。方法根据患者意愿,采用不同治疗方法,将100名中低位直肠癌患者随机分成2组。治疗过程中,采用国产一次性双吻合器治疗的患者归为对照组(52例);采用弧形切割闭合器治疗的患者记为观察组(48例)。观察2组患者保肛成功率,术中、术后相关参数及术后并发症等指标,探讨其临床治疗价值。结果观察组保肛成功率明显高于对照组,P<0.05。观察组患者手术时间、术后引流时间、导尿管拔出时间及术后2个月内每日大便次数均少于对照组,组间差异有统计学意义(P<0.05)。观察组术后吻合口瘘、伤口感染、炎性肠梗阻发生例数均略低于对照组,但差异无统计学意义(P>0.05);2组患者肺部感染、盆腔脓肿发生例数组间差异无统计学意义(P>0.05)。结论弧形切割闭合器应用于结直肠癌保肛手术安全可靠,且保肛成功率高,值得临床推广应用。更多还原

【Abstract】 Objective To investigate the clinical effect of curved cutter stapler in sphincter-preserving surgery for middle and low rectal cancer. Methods 100 patients with middle and low rectal cancer were randomly divided into 2 groups according to patients’ wishes and Different treatment methods were used. The control group( 52 cases) were given domestic disposable double stapler treatment; The observation group( 48 cases) were given the curved stapler treatment. The success rate of anus preservation,intraoperative and postoperative parameters and postoperative complications and other indicators of the 2 groups were observed. Results The success rate of the observation group was significantly higher than that of the control group,P < 0. 05; The operation time,postoperative drainage time,the time of catheter drainage and the daily stool frequency in the 2 months after operation were less than those of the control group,P < 0. 05; The postoperative anastomotic leakage,wound infection,inflammatory bowel obstruction occurred in the observation group were little lower than the control group,P > 0. 05; There was no significant difference between the 2 groups in pulmonary infection and pelvic abscess( P > 0. 05). Conclusion Curved cutter stapler in sphincter-preserving surgery for middle and low rectal cancer is safe and reliable,and has high success rate,it is worthy of clinical application.更多还原
