眼底荧光造影及三维光学相干断层扫描评估巩膜扣带术后视物变形患者黄斑特征
The application of fundus fluorescein angiograph and three-dimensional optical coherence tomography to the macular character in eyes with metamorphopsia after scleral buckling surgery
【摘要】 目的:应用眼底荧光造影(FFA)及三维光学相干断层成像(3D-OCT)检查巩膜扣带术后视物变形患者黄斑特征,评价黄斑形态变化对视功能的影响。方法:选择累及黄斑的孔源性视网膜脱离患者行巩膜扣带手术治疗,随访6个月仍主诉视物变形患者53例53只眼,利用FFA及3D-OCT检查黄斑区,分析黄斑形态变化与视功能的关系。结果:术前与术后6月裸眼视力比较差异无统计学意义,术前与术后6月最佳矫正视力比较差异无统计学意义。49眼存在黄斑区视网膜下积液(92.5%),所有患眼均可见光感受器细胞的内节与外节(IS/OS)反射不连续,有中断现象。结论:累及黄斑的孔源性视网膜脱离患者接受巩膜扣带术后视物变形的存在与IS/OS反射不连续有关,持续的黄斑区视网膜下液的存在对视功能也造成影响。FFA的结果与3D-OCT的结果并不完全对应。
【Abstract】 Objective:To evaluated the influence of the macular morphological changes to visual function through the application of fundus fluorescein angiograph and three-dimensional optical coherence tomography to the macular character in eyes with metamorphopsia after scleral buckling surgery.Methods:Macula-off rhegmatogenous retinal detachment in our hospital underwent scleral buckling surgery.In the follow-up 6 month,53 patients(53 eyes)with metamorphopsia after the successful scleral buckling surgery were included in the study.These patients accepted macular check using the fundus fluorescein angiograph and three-dimensional optical coherence tomography.The relation between macular character changes and vision function was summarized.Results:The difference between the postoperative natural visual acuity in the follow-up 6 month and the preoperative natural visual acuity was no significant.The difference between the postoperative best corrected vision acuity in the follow-up 6 month and the preoperative best corrected vision acuity was no significant.In the follow-up,the persistent localized submacular fluid could be found in 49 eyes(92.5%),all cases detected the disruption of the inner segment and outer segment junction of retinal photoreceptor cells.Conclusion:The metamorphopsia is related to the disruption of photoreceptor inner segment and outer segment after the successful scleral buckling surgery for macula-off rhegmatogenous retinal detachment.The persistent localized submacular fluid also can influence vision function.The results are not exactly corresponding between the fundus fluorescein angiographand and the three-dimensional optical coherence tomography.
