膝关节假体在人工全膝关节置换术修复中的应用效果及髌骨轨迹不良发生状况分析
The application effect of high flexion knee prosthesis in total knee arthroplasty and the incidence of patellar maltracking
【摘要】 目的探究高屈曲型膝关节假体在人工全膝关节置换术修复中的应用效果及其髌骨轨迹不良发生情况。方法按随机数字表法分为高屈曲组89例(126膝)和对照组67例(95膝)。对照组采用普通非高屈曲型假体进行初次人工全膝关节置换术,观察组患者则采用高屈曲型假体进行初次人工全膝关节置换术。于手术前后对比2组患者的美国膝关节协会制定的评分标准(KSS)评分及最大屈曲角度,并于术后12个月对2组患者进行X线复查,测量膝关节活动范围,观察比较其髌骨、下肢力线以及假体情况。结果术后3、6、12个月,高屈曲组的KSS评分和最大屈曲角度均明显高于对照组(P<0.01)。术后12个月,高屈曲组的膝关节活动范围明显高于对照组,在髌骨轨迹不良、下肢力线较差以及假体不稳定等事件的发生上,高屈曲组患者则远远低于对照组(P<0.05)。结论在人工全膝关节置换术中应用高屈曲型膝关节假体,其术后膝关节功能、膝关节活动范围均优于普通非高屈曲型假体,能够获得最大屈曲角度,同时其髌骨轨迹不良等事件的发生率也低于普通非高屈曲型假体。
【Abstract】 Objective To investigate the application effect of high flexion knee prosthesis in total knee arthroplasty and the incidence of patellar maltracking.Methods According to random number table,the 156 patients were divided into the high flexion group(89cases,126knees)and the control group(67cases,95knees).The control group was treated with conventional non-high flexion prosthesis for primary total knee arthroplasty;while the patients in the observation group were treated with high flexion prosthesis for primary total knee arthroplasty.We compared the KSS score and maximum flexion angle in two groups before and after operation.12 months after operation,the patients of two groups underwent X-ray examination,and were measured the range of motion of the knee joint.And we compared the patella,lower limb strength and prosthesis in two groups as well.Results At 3,6and 12 months after operation,the KSS score and the maximum flexion angle in high flexion group were significantly higher than those in the control group,and the differences were statistically significant(P<0.01).At the same time,the incidence of patellar maltracking,lower extremity stress line and prosthesis instability were far lower than those of the control group(P <0.05).Conclusion In total knee arthroplasty,the postoperative knee function and knee range of activities of high flexion knee prosthesis are better than those of ordinary non-high flexion prosthesis.It can get the maximum flexion.At the same time,the incidence of patellar maltracking was lower than that of normal non-high flexion prosthesis.
