【摘要】 [目的]调查广东省各级医院护士用药错误现状及其影响因素,为制定用药错误的护理质量改善指引提供依据。[方法]采用便利抽样方法抽取广东省一级、二级、三级医院90家,采用自行设计的"护士用药错误影响因素调查表"对临床一线的1 528名护士进行问卷调查。[结果]231名(15.12%)护士过去1年内发生过用药错误,共计441例次;排名前5位的分别为用药时间错误、剂量错误、漏打漏发、病人错误、药物错误;医院级别、职称、工作环境、医院的发药系统、使用掌上电脑(PDA)扫描核对技术、主动上报、惩罚方式、整改分析是护士用药错误的主要影响因素,其中医院的发药系统、工作环境、医院级别及职称是主要影响因素;使用PDA扫描核对技术、建立主动上报、适当的惩罚及整改分析机制是护士用药错误的保护性因素。[结论]护士发生用药错误与多种因素有关,医院管理者应针对性地制定用药安全质量改进指引,降低护士用药错误,保障病人用药安全。更多还原

【Abstract】 Objective:To probe into the status quo and influencing factors of nurses’ medication errors in hospitals at all levels in Guangdong Province,and to provide referrences for the improvement of nursing quality.Methods:A convenient sampling method was used to select 90 hospitals in Guangdong province,including grade one,grade two and grade three hospital,and a self-designed questionnaire was used to investigate the influence factors of nurses’ medication errors in the first line of the questionnaire.The questionnaire survey was conducted for 1 528 nurses.Results:231nurses(15.12%)over the past 1year had occurred in the medication errors,a total of 441cases;the top 5were medication time wrong,wrong dose,medication time,missed leaks,patient’s errors,medication errors;hospital level,job title,working environment,hospital medicine system,using the PDA scan check technology,take the initiative to report and the punishment and rectification analysis were the main factors affecting nurses’ medication errors,the hospital dispensing system,working environment,hospital level and title were the main influence factors;using PDA scanning check technology,the establishment of check active reporting,appropriate punishment and rectification of mechanism were the protective factors of nurses medication errors.Conclusions:There were many factors related to the occurrence of medication errors in hospital.The hospital managers should formulate guidelines for medication safety and quality improvement,reduce medication errors and ensure the safety of patients.更多还原
