超声射频数据处理技术评价强直性脊柱炎患者颈总动脉内-中膜厚度和血管弹性

Evaluation of common carotid artery intima-media thickness and elasticity in patients with ankylosing spondylitis by ultrasound radio frequency data technology

【摘要】 目的探讨超声射频数据处理(RF-data)技术评价强直性脊柱炎(AS)患者颈总动脉内-中膜厚度(IMT)和血管弹性的价值,并分析其与临床资料间的相关性。方法选取2015年8月—2016年5月间在上海交通大学附属第六人民医院明确诊断为AS的患者60例,以及同期行体格检查的健康志愿者30名。利用RF-data技术测量所有研究对象的颈总动脉IMT、颈总动脉扩张值(CD)、脉搏波传播速度(PWV)、硬度指数(β)。比较两组间患者的一般资料、颈总动脉IMT、CD、PWV、β值,以年龄、病程、BMI、CRP、红细胞沉降率(ESR)为变量,采用Pearson分析AS患者颈总动脉IMT和PWV与各变量间的相关性。结果 AS组的CRP水平和ESR均显著高于正常对照组(P值均<0.01)。AS组左、右侧颈总动脉的IMT、PWV、β值均显著高于正常对照组同侧(P值均<0.05),CD显著低于正常对照组同侧(P值均<0.05)。AS患者的颈总动脉IMT和PWV与年龄(r=0.386和0.425)、病程(r=0.459和0.528)、ESR(r=0.276和0.316)均呈正相关(P值均<0.05),与BMI(r=0.225和0.241)、CRP(r=0.196和0.218)均不相关(P值均>0.05)。结论 RF-data技术可自动、准确地检测颈总动脉IMT和血管弹性参数的变化。AS患者颈总动脉IMT、PWV和β值均显著高于正常人,上述参数随着患者的年龄、病程、ESR增加而增加。

【Abstract】 Objective To explore the value of ultrasound radio frequency data(RF-data)technology in assessment of carotid intima-media thickness(IMT)and arterial elasticity in patients with ankylosing spondylitis(AS)and to evaluate its influence factors.Methods From August 2015 to May 2016,60 AS patients and 30 healthy volunteers were randomly selected from Shanghai No.8 People’s Hospital.The IMT,carotid distention(CD),pulse wave velocity(PWV)and stiffnessβof the common carotid artery(CCA)were measured in all the subjects by RF-data technique.Clinical data,IMT,CD,PWV,andβvalue were compared between AS patients and healthy volunteers.The correlation between IMT,PWV and age,course of disease,body mass indes(BMI),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR)in AS patients was analyzed by Pearson correlation analysis.Results CRP and ESR of AS group were significantly higher than those of control group(both P<0.01).The bilateral IMT,PWV andβvalue in the AS group were significantly higher than those in the ipsilateral control group(all P<0.05),while the CD were significantly lower than those in the ipsilateral control group(both P<0.05).The IMT and PWV of the CCA in AS patients were positively correlated with age(r=0.386 and 0.425),course of disease(r=0.459 and 0.528),and ESR(r=0.276 and 0.316,all P<0.05),but were not related to BMI(r=0.225 and 0.241)or CRP(r=0.196 and 0.218,all P>0.05).Conclusion RF-data can accurately evaluate the increased IMT of CCA and decreased arterial elasticity in AS patients.IMT,PWV andβvalues in AS patients are significantly higher than those in healthy controls.IMT and vascular stiffness are positively related to the age,course of disease and ESR.
