生食泥鳅引发的寄生虫感染的诊治探讨

【摘要】 目的探讨因生食泥鳅所致寄生虫感染的诊断治疗,以提高临床诊治水平。方法对因生食泥鳅引发的寄生虫感染病例的临床资料进行回顾性分析。结果收集临床资料较为完整的病例332例,81.33%发病时间为19~60d;62.95%临床表现仅为畏寒、发热;98.49%出现外周血中嗜酸性细胞增高;全部病例寄生虫抗体检查至少1项或多项结果为阳性;病原学检查仅有6例发现虫体或虫卵;腹部B超检查51.44%有阳性发现;治疗过程中22.60%出现发热的类赫氏反应;经吡喹酮治疗后332例全部达到临床治愈。结论生食泥鳅感染寄生虫病已得到证实,可能感染的寄生虫病有多种,包括棘口吸虫、棘颚口线虫、华支睾吸虫、肺吸虫等。取得病原学的确诊依据应成为今后该项工作的重点。

【Abstract】 Objective To explore the diagnosis and treatment of parasitic infection caused by eating raw loach,in order to improve the level of clinical diagnosis and treatment. Methods Clinical data of cases of parasite infection caused by eating raw loach was retrospectively analyzed.Results Complete clinical data were collected from 332 patients,and the onset time was from19 to 60 d in 81. 33% of patients.The clinical symptoms of 62. 95% of patients only performed chills and fever. In 98. 49% of the patients,the eosinophil cells were increased in peripheral blood.All cases of parasite antibodies were positive for at least 1 or more of the results.Only 6 cases were found worms or eggs by the etiological examination.There were positive findings in 51. 44% of patients by abdominal B ultrasound examination.Herxheimer-liked fever reaction occurred in 22. 60% of patients during the treatment.332 cases all achieved clinical cure after the treatment of praziquantel. Conclusions Parasite infection by eating raw loach has been confirmed. There are a variety of parasitic diseases which may be infected,including echinostoma,gnathostoma,clonorchis sinensis,paragonimiasis and so on.And it should be the focus of the work in the future to get the definite diagnosis basis of etiology.
