掌侧经皮可埋头空心螺钉内固定治疗舟骨骨折的疗效评价

【摘要】 目的探讨掌侧经皮可埋头加压空心螺钉通过内固定手术的方式治疗新鲜不稳定型腕舟状骨骨折的的手术方法,并通过相应的评估手段及辅助检查评价其疗效。方法回顾性分析2008年9月至2016年4月采用3.0 mmAO可埋头空心加压螺钉经皮微创治疗12例新鲜舟状骨骨折患者的病例资料,所有病例的术前评估均通过采用腕关节X线摄片和CT断层扫描来完成。手术方式选择闭合复位掌侧经皮可埋头加压空心螺钉内固定术。术后采用患手石膏固定2周后开始行功能锻炼。结果所有患者均获得平均6~24个月的随访,所有患者伤处骨折均达到良好的骨性愈合,骨折愈合周期约为4~8个月,平均5个月。按Mayo临床评分系统进行评定:优8例,良4例。结论掌侧经皮可埋头空心加压螺钉治疗新鲜不稳定型舟状骨骨折可以使骨折断端得到很好的加压,手术过程简便,疗效确切,深受广大医务人员的认可。

【Abstract】 Objective To introduce the surgical method and evaluate the efficacy of volar percutaneous headless compression screw internal fixation for the treatment of New scaphoid fractures,and evaluate the curative effect by means of corresponding evaluation methods and auxiliary examination. Methods From September 2008 to April 2016,12 cases with acute scaphoid fracture treated by percutaneous headless compression screw internal fixation were studied retrospectively. All cases are evaluated the displacement,and the shape of fracture fragments by X-ray and CT before fixing the fracture. After giving thumb spica cast immobilization for 2 weeks,rehabilitation exercises were performed.Results All patients were followed up with 6 ~ 24 months. All fractures ’ healing time was 4 ~ 8 months,with average time of 5 months. The modified Mayo wrist score were improved from preoperative after operation; the results were excellent in 8 cases and good in 4 cases. Conclusions Volar percutaneous headless compression screw internal fixation for the treatment of New scaphoid fractures is an surgical technique with efficacy and safety,recognized by the majority of medical staff.
