【摘要】 目的:比较腔镜腋窝淋巴结清扫术与常规腋窝淋巴结清扫术的优缺点。方法:将2013年2月-2016年10月,我科收治临床Ⅰ、Ⅱ期乳腺癌病例中68例腔镜腋窝淋巴结清扫术与80例常规腋窝淋巴结清扫术的临床资料进行前瞻性研究,比较两组手术时间、术中出血量、清扫淋巴结数目、住院时间以及术后并发症的发生率。结果:腔镜组手术时间(182.3±20.4)min显著长于常规组(127.5±15.7)min(P=0.001),腔镜组术中出血量(88.3±17.5)ml显著少于常规组(136.1±23.4)ml(P=0.003),两组淋巴结清扫数目差异无统计学意义(P=0.876),腔镜组术后并发症少于传统组(P=0.034),随访3月至38月,癌细胞无局部复发及远处转移。结论:乳腺癌腔镜腋窝淋巴结清扫术具有创伤小、并发症少、切口小等优点,值得临床应用。更多还原

【Abstract】 Objective: To examine the merits and demerits of clinical application of mastoscopic axillary lymph node dissection and traditional operation. Methods:Perform the prospective study of clinical data of patients with breast cancer in our hospital from February 2013 to October 2016,of which 68 patients(at stage I or II) were treated by mastoscopic axillary lymph node dissection and 80 were treated by traditional operation.Contrast the operation time, bleeding, number of lymph node removed, length of hospitalization and incidence of postoperative complications. Results:The operation time of mastoscopic group(182.3 ±20.4) minutes were longer than that of traditional operation group(127.5 ±15.7)minutes(P=0.003).The bleeding of mastoscopic group(88.3 ±17.5)ml were less than that of traditional operation group(136.1±23.4)ml(P=0.003). As for the number of lymph nodes removed, the difference between mastoscopic group(20.7±3.6) and traditional operation group(20.3±4.9)was of no statistical significance(P=0.876). The incidence of postoperative complication was less than that of traditional operation group(P=0.034). The follow-up examinations in next three to thirty-eight months revel no local recurrence and distant metastasis. Conclusion:Mastoscopic axillary lymph node dissection is of clinical significance in patient with breast cancer for less trauma,less complications,smaller incisions,and quicker recovery. Time for learning and practicing is demanded for mastoscopic technique mastery,it will otherwise cause prolonged operation time.更多还原
