PCOS不孕患者性激素与Th1/Th2细胞因子的相关性研究

【摘要】 目的探讨多囊卵巢综合征(polycystic ovary syndrome,PCOS)不孕患者性激素与外周血Th1/Th2细胞因子的相关性。方法将2013年3月至2014年12月于我院诊疗的PCOS不孕患者105例作为观察对象,选择75例同期就诊于我院的正常非妊娠妇女作为对照组。于月经周期第2~4天抽血行性激素检查;使用流式细胞术检测外周血Th1(IFN-γ)、Th2(IL-4)细胞因子。结果 PCOS不孕患者外周血中,Th1细胞因子(IFN-γ)比例显著高于对照组(分别为24.54%±9.53%和14.64%±7.33%,P<0.05);Th2细胞因子(IL-4)比例2组无显著差异(分别为1.44%±1.53%和1.23%±1.26%,P>0.05);Th1/Th2比值PCOS组显著高于对照组(分别为44.21%±49.91%和20%.44±18.15%,P<0.05)。在对照组Th1/Th2与FSH的相关性有统计学意义(r=0.534,P<0.000 1);PCOS患者中,HA组Th1/Th2与睾酮(T)的相关性有统计学意义(r=0.652,P<0.000 1)。结论 PCOS患者体内高雄激素血症可能引起外周血Th1/Th2比例失调。

【Abstract】 This study was performed to explore the association between Th1/Th2 cytokines levels and sexhormones levels in polycystic ovary syndrome(PCOS) dysgenesis patients. Total of 105 cases of dysgenesis patientsaccepting treatment in our hospital during March 2013 to December 2014 were recruited as the observation casesand 75 cases of nonpregnant normal women were selected as control group. Then hormones and peripheral bloodTh1(IFN-γ)/Th2(IL-4) cytokines were measured with flow cytometry in the menstrual cycle line 2-4 days. Datashowed that Th1 cytokine(IFN-γ) proportion in PCOS group was significantly higher than that in control group(24.54±9.38 and 14.64±7.33, P<0.05); Th2 cytokine(IL-4) in PCOS group and control group was not significantdifference(1.44±1.53 and 1.23±1.26, P>0.05); Th1/Th2 proportion was significantly higher in PCOS group than thatin control group(44.21±49.91 vs 20.44±18.15, P<0.05). In control group, the correlation between Th1/Th2 and FSHwas statistical significance(r=0.534; P<0.000 1); while in PCOS group, significant correlation between Th1/Th2 andtestosterone(T) was found in hyperandrogenism cases(r=0.652;P<0.000 1). In conclusion, immunologic derangementof Th1/Th2 proportion is found in most PCOS patients, which probably causes by hyperandrogenism.
