改良三点式微创皮下隧道重睑术的临床应用

【摘要】 目的:探索改良三点式微创皮下隧道重睑术的临床应用效果。方法:收集2015年8月至2016年5月在中国医学科学院整形外科医院整形七科行改良三点式微创皮下隧道重睑术患者30例。所有患者术前标记三处切口点,设计重睑线。局麻成功后,于标记点切开眼睑皮肤及皮下组织,在皮下形成一个贯通隧道,去除部分皮下组织、眼轮匝肌及脂肪。外侧切口根据需要设计成梭形,以去除部分皮肤,达到有限解决眼睑松弛。双极电凝止血后,三点小切口分别与睑板前筋膜缝合固定,术后7d拆线。结果:随访3~12个月,患者均在1~2周内消肿,重睑线形成持久、自然,且瘢痕不明显,患者满意度高。结论:改良三点式微创皮下隧道重睑术是具有创伤小、恢复快、效果持久且自然、更灵活等优点,值得临床应用推广。

【Abstract】 Objective To explore clinical applications of modified three minimally invasive incision double eyelid plasty under subcutaneous tunnel. Methods There were 30 cases performed with modified three minimally invasive incision under subcutaneous tunnel for double eyelid plasty in the 7th department of Plastic Surgery Hospital of Chinese Academy of Medical Science from Aug.2015 to May.2016. Eyelid lines were designed and marked in three points. Incising the skin and subcutaneous tissue to form a through tunnel under local anaesthetic,then snipping the orbicular muscle and excess fat.The lateral incision was designed into spindle shape if needed,to remove part of the skin and solve palpebral relaxation to a certain extent. Then sewed up the mini incision with the tissue front of tarsal plate after bipolar coagulation hemostasis. Removed stitches for 7 days after the operation. Results All the cases were followed up for 3 to 12 months after operation.They were all detumescence within 1-2 weeks,and had high satisfaction with lasting and natural double eyelid shape. Conclusion This technique has advantages of less trauma,quicker recovery,lasting and natural effect,and more flexibility.So,it can be widely conducted in clinic.
