【摘要】 目的对比探讨经尿道输尿管硬镜在女性2~2.5cm肾盂结石手术中较经尿道输尿管软镜及经皮肾镜钬激光碎石术的临床应用优势及可行性。方法回顾性分析潍坊医学院附属医院泌尿外科2014年3月~2016年5月收治的90例女性2~2.5cm肾盂结石患者的临床资料。随机分为实验组和对照组,实验组使用经尿道输尿管硬镜的手术方式进行治疗,而对照组则采用经皮肾镜、经尿道输尿管软镜钬激光碎石,对照比较3组患者在结石清除率、手术不良反应、手术时间、手术费用、住院时间方面的临床差异。结果经尿道输尿管硬镜在手术时间、手术费用、出血、术后并发症及住院时间方面明显较经皮肾镜、经尿道输尿管软镜低,差异有统计学意义(P<0.05)。输尿管硬镜、输尿管软镜的结石清除率较经皮肾镜稍低,石街发生率也稍高于经皮肾镜,但差异无统计学意义。结论女性2~2.5cm肾盂内结石,经尿道输尿管硬镜钬激光碎石术可作为首选手术方式,在个别治疗失败的患者中可改行经皮肾镜碎石术或行经尿道输尿管软镜做术前准备。更多还原

【Abstract】 Objective To compare the clinical advantages and feasibility of transurethral ureteroscopic lithotripsy in the treatment of 2 to 2.5cm renal pelvis stones by holmium laser lithotripsy.Methods The clinical data of 90 female patients with 2~2.5cm renal pelvis calculi who were treated in the department of urology of the affiliated hospital of weifang medical university from March 2014 to May2016 were retrospectively analyzed.They were randomly divided into experimental group and control group,the experimental group was treated with transurethral Ureteroscopy,while the control group underwent the percutaneous nephrolithotomy,transurethral ureteroscope holmium laser lithotripsy were compared,compared three groups of patients in the stone clearance rate,adverse reaction,operation time,operation expenses,and hospitalization time of clinical differences.Results By comparison of transurethral ureteroscopic surgery at the time,the cost of surgery,bleeding,postoperative fever and hospitalization time compared with percutaneous nephrolithotomy,transurethral ureteroscope was lower,the difference was statistically significant,ureteroscopic stone,flexible ureteroscope in percutaneous nephrolithotomy rate was slightly lower,and the incidence was also slightly higher than that of percutaneous nephrolithotomy,but the difference was not statistically significant.Conclusion Transurethral ureteroscopic holmium laser lithotripsy can be used as the preferred mode of operation to treat 2~2.5cm female pelvis calculi,it can be diverted to percutaneous nephrolithotomy or transurethral ureteroscope for preoperative preparation in individual patients failed.更多还原
