妊娠期糖尿病孕妇中视黄醇结合蛋白4的表达与胰岛素抵抗的关系

【摘要】 目的探讨妊娠期糖尿病(GDM)孕妇血清及胎盘组织中视黄醇结合蛋白4(RBP4)的表达水平与胰岛素抵抗的关系。方法用酶联免疫吸附法检测妊娠期糖尿病孕妇32例(GDM组)、糖耐量正常孕妇30例(正常妊娠组,对照组)血清中RBP4水平,用放射免疫法及葡萄糖氧化酶法检测两组孕妇空腹胰岛素(FINS)及空腹血糖(FPG)水平,并计算稳态模型的胰岛素抵抗指数(HOMA-IR)。采用荧光定量PCR技术及蛋白免疫印迹法检测两组孕妇胎盘组织中RBP4 mRNA和蛋白表达水平,并对GDM组孕妇胎盘组织中RBP4 mRNA和蛋白表达与血清中RBP4水平及HOMA-IR行相关性分析。结果 1GDM组孕妇血清中RBP4,FINS,FPG水平及HOMA-IR均高于对照组,差异有统计学意义(P均<0.05)。2GDM组孕妇胎盘组织中RBP4 mRNA和蛋白表达水平高于正常妊娠组,差异有统计学意义(P均<0.05)。3GDM组孕妇胎盘组织中RBP4 mRNA和蛋白表达水平与HOMA-IR均有明显相关(r=0.541,P<0.05);血清中RBP4水平与HOMA-IR呈明显相关(r=0.793,P<0.05);胎盘组织中RBP4 mRNA和蛋白表达水平与血清中RBP4水平无明显相关性(P>0.05)。结论妊娠期糖尿病孕妇出现的胎盘组织中RBP4 mRNA高表达及血清中RBP4高水平的变化,可能是妊娠期糖尿病孕妇发生胰岛素抵抗的机制之一。

【Abstract】 Objective To investigate the correlation of the expression of RBP4 in serum and placental tissue with insulin resistance in gestational diabetes mellitus.Methods Thirty-two women with gestational diabetes mellitus were recruited for gestational diabetes mellitus group.And 30 healthy pregnant women were taken as control group.Enzyme-linked immunosorbent assay(ELISA) was used to measure the plasma concentrations of RBP4 in these women.Radio immunity assay was used to measure the plasma levels of fasting insulin(FINS).Fasting plasma glucose(FPG) was measured by glucose oxidase,and calculated the home model insulin resistance index(HOMAIR).Fluorogenic quantitative polymerase chain reaction and Western blot were used to investigate the expression of RBP4 mRNA and protein in placental tissue.Then correlation analysis of the expression of RBP4 mRNA and protein in placental tissue of the gestational diabetes mellitus group with the plasma concentrations of RBP4 and HOMA-IR were carried out.Results 1The plasma concentrations of RBP4,FINS,FPG,HOMA-IR in gestational diabetes mellitus group were significantly higher compared with control group(P<0.05).2The expression of RBP4 mRNA and protein in placental tissue of the gestational diabetes mellitus group were significantly higher than that in control group(P<0.05).3In gestational diabetes mellitus group,the levels of RBP4 mRNA and protein in placental tissue were positively correlated with HOMA-IR(r=0.541,P<0.05);The plasma concentrations of RBP4 were positively correlated with HOMA-IR(r=0.793,P<0.05);There were no correlations of the levels of RBP4 mRNA and protein in placental tissue with the plasma concentrations of RBP4.Conclusion High expression of RBP4 mRNA in placental tissue and the elevation of the levels of serum RBP4 in gestational diabetes mellitus may be one of the mechanisms of insulin resistance.
