【摘要】 目的本研究拟探讨血浆脂蛋白相关磷脂酶A2(lipoprotein-associated phospholipase A2,Lp-PLA2)活性与冠状动脉粥样硬化性心脏病(冠心病)血管病变支数的相关性。方法共入选212例经冠状动脉造影检查明确冠状动脉狭窄严重程度的患者,抽取其空腹静脉血用于检查血浆Lp-PLA2活性和血脂等,同时记录研究对象人口学资料、既往病史、服药情况及体格检查指标。依据血浆Lp-PLA2活性将所有研究对象分为4组,进行组间比较。结果 (1)与其他3组相比,血浆Lp-PLA2活性最高组吸烟和糖尿病患者比例较高,三支血管病变和左主干加三支血管病变患者比例也较高,差异均有统计学意义(P<0.05)。(2)Pearson相关分析提示血浆Lp-PLA2活性与吸烟、总胆固醇和低密度脂蛋白-胆固醇浓度、2型糖尿病以及冠状动脉狭窄支数呈正相关,而与高密度脂蛋白-胆固醇浓度和服用他汀类药物呈负相关。(3)采用Logistic回归分析血浆Lp-PLA2活性与冠心病血管病变支数相关性,在校正年龄、性别、吸烟、酗酒、原发性高血压(高血压)、糖尿病、总胆固醇等冠心病传统危险因素后,血浆Lp-PLA2活性升高与冠心病血管病变支数程度仍呈正相关,危险比为1.45(95%置信区间为1.23~2.02)。结论血浆Lp-PLA2活性与冠心病患者冠状动脉狭窄支数具有独立相关性。更多还原

【Abstract】 Objectives To investigate the relationship between serum lipoprotein-associated phospholipase A2(Lp-PLA2)activity and number of coronary artery stenosis. Methods A total of 212 participants who undergone coronaryangiography were enrolled and fasting venous blood was drawn for evaluating serum Lp-PLA2 activity and lipidconcentrations. Demographic data,previous medical history,medication usage and anthropometric indexes werecollected. Between-group comparisons were performed according to the quartile of serum Lp-PLA2 activity.Results(1)Compared to the other 3 groups,the proportion of smokers and participants with diabetes mellitus,withthree-vessels stenosis and three-vessels plus left-main artery stenosis were significantly higher in the highest quartilegroup(P<0.05).(2)As shown by the Pearson correlation analysis,serum Lp-PLA2 activity positively associated withsmoking,serum concentrations of total cholesterol and low-density lipoprotein cholesterol and diabetes mellitus,whereas negatively associated with high-density lipoprotein cholesterol and statins.(3)Logistic regression analysisshowed that after adjusted for age,sex,smoking,alcohol abuse,hypertension,diabetes mellitus and total cholesterol,serum Lp-PLA2 activity remained significantly associated with the number of coronary artery stenosis,with odds ratioof 1.45 and 95 % confidence interval of 1.23-2.02.
